Hospital of our client in Connacht
The paediatric
haematology department at the hospital provides all aspects of clinical
haematology and laboratory care to the paediatric population of Connacht and
West / North West of the Republic of Ireland. The department is comprised of a
Treatment Centre for malignant haematology; JACIE accredited Stem Cell
Transplant Unit; Regional Haemoglobinopathy centre; and Paediatric Haemophilia
Comprehensive care centre. It is envisaged therefore that in addition to the
above subspecialty responsibilities that the new post holders will also
contribute to additional subspecialties, according to their interests and the
needs of the department, to include benign, malignant and laboratory
haematology.
Haematology
/ Paediatric Haematology Department
The Haematology
Department at the Hospital is the regional and tertiary referral centre for
children and young people with malignant and chronic blood disorders, providing
a comprehensive paediatric laboratory and clinical haematological service to
the hospital and to the region. It works closely with the paediatric oncology
department at hospital and manages its own departmental governance and finance
within the BSC (Blood, Stem cell, Cancer) board group. The department is
managed within the Division of Neurosciences and Haematology/Oncology.
The haematology department manages all
aspects of malignant, benign and haemostasis/ thrombosis paediatric
haematology. It is one of the largest Principle Treatment Centres (PTC) for
paediatric haemato-oncology in the region; a JACIE accredited centre for Stem
Cell Transplant; regional centre for haemoglobinopathy; and Paediatric
Haemophilia Comprehensive Care Centre.
The department has strong research links with the
Universities. There is a well-established infrastructure for the management and
provision of early phase clinical trials, in which the department has a
particular interest.
Consultants:
The
new post-holders will join the team of 5 established paediatric haematologists
at the University Hospital. The current consultant team have subspecialty
interests as detailed below. Departmental roles are rotated through the
consultant team according to individual interests with the clinical lead role
usually held for a maximum of 3 years at one time. Several of the consultants
also have external/ National roles to complement their local roles and
responsibilities.
Middle grade
provision:
Funding is provided for up to 10 middle grade (registrar
level) training posts in haemato-oncology (divided as in malignant haematology,
benign haematology, 2 in SCT, 3 in oncology). At present, 3 haematology
trainees are provided from the CPSP / HSE Rotation Haematology at any one time,
on a rotational basis. At least 1-2 of these trainees may be specialist
paediatric haematology trainees who will spend a minimum of 2 years at the
hospital. Adult haematology trainees are fully integrated into ward work and
on-calls and spend 6 months at the hospital for paediatric haematology
training. There is provision for 3 paediatric oncology trainees at one time.
These may be oncology trainees (usually 1-2) or paediatricians in training,
often with a special interest in shared-care haemato-oncology. Further
middle-grade posts are filled by overseas clinical fellows and we have
established a highly successful fellowship programme where fellows from all
over the world have come to train with us for up to 2 years.
Additional
medical and nursing staffing:
Three SHO doctors are rotated to haematology/oncology for 6 month blocks as
part of the regional paediatric training posts. Additional support is provided
by 2 acute ward based Advanced Nurse Practitioners (ANPs), 4 outpatient based
specialist ANPs (in leukaemia, haemoglobinopathy, haemostasis and apheresis)
and by a specialist anticoagulation nurse.
Specific Duties
1 (10 PA) consultant post is proposed. One is a replacement post for the current post holder who is retiring, and would include subspecialist responsibility for haemostasis/ thrombosis together with the haemophilia centre lead. The other is a newly created post with proposed subspecialist responsibility for Stem Cell Transplantation and apheresis, together with the existing SCT consultant team. Further subspecialty responsibilities are expected to be taken on by the successful candidate(s) according to their interest and the departmental needs, but it is envisaged that at least one of the posts will include malignant haematology as a subspecialty interest. Other areas for consideration and development would include general/ benign haematology. Further discussions between prospective candidates and the existing consultant team of subspecialty areas of interest and for development would be welcomed. There is ample opportunity for research and teaching.
The unit operates a consultant
led service, with the on-service consultants undertaking a minimum of 2
consultant-led ward-rounds per week, and otherwise readily available for
patient review and clinical advice. Two handovers are performed each day, involving
on-service consultants, middle-grade doctors and senior nursing staff from the
ward and day-unit, to ensure a high level continuity of care. After-hours care
(1730-2030) is provided by one of the haemato-oncology registrar level doctors,
with the assistance of an SHO doctor. Following that, there is a hospital at
night cover system of one covering SHO doctor, supported by an RMO (Medical
Officer). Two consultants are on-call from home (one oncology consultant, one
haematology consultant), and should always be readily available by telephone
and prepared to come into the hospital as necessary.
The majority of outpatient
clinics are consultant led (exceptions being the ANP led vincristine clinic and
the ANP led haemoglobinopathy clinic) and each consultant has a clinic each
week in their sub-speciality interest (equating to 2 clinics per week). At
present the clinics are continued when the consultant is on service, however it
is envisaged that with the expansion of the haematology consultant numbers that
at least one clinic per week will be cancelled when on-service.
Practical procedures of bone
marrow aspirates and intrathecal chemotherapy are rarely performed by the
haematology consultants, however it is expected that these skills are
maintained and each consultant is expected to be re-accredited to prescribe and
administer intrathecal chemotherapy each year. Bone marrow harvests are
undertaken by the SCT consultant team.
All haematology consultants are
expected to cover the laboratory service, for the reporting of blood films and
bone marrows, on a rotational basis.
On-call is run as a first-on and
second-on system with the clinical oncologists. With haematology consultants in
post this would equate to a 1:7 weekday first on-call, 1:7 weekday second
on-call, 1:7 weekend first on-call, and 1:7 weekend second on-call. Theweekend
on-call service comprises 2 consultants, a middle-grade (registrar level)
doctor (0830-2030) and a SHO doctors (0830-2030). Both consultants meet for
handover at 0900 with the middle-grade doctors, followed by a clinical review
of all the ward patients, with the second-on consultant handing over and
leaving at 1300 and the first-on consultant finishing when the ward-round and
reviews are completed. The haematology consultant also covers the laboratory
for anticoagulation advice and dosing.
All consultants are responsible
for the professional supervision and management of junior medical staff. If
appropriate, the successful candidates will also be planned to take on the role
of education supervisor for a named junior doctor on rotation.
All consultants are expected to
engage with continuing medical education, the trust’s Moodle updates and
clinical audit within the department.
Cross-cover of consultant
colleagues is expected for periods of annual/ study leave and sickness.
Teaching Commitments

and 4th & 5th year medical students.

teaching of other groups as required including other
specialty juniors and specialist nurses.
Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Paediatric Consultant, on behalf of the Trust’s Medical
Director, based on the provisional timetable. This will be signed by the Chief
Executive and will be effective from the commencement date of the appointment.
It will be reviewed at 3 months & thereafter
annually or at any time, but no less than 3 months after a previous review, as
requested by the appointee, Operational Medical Director or Clinical Lead &
adjusted accordingly to the agreement of both parties.

Standard Duties and responsibilities
a) To
participate in development of and undertake all duties and functions pertinent
to the Consultant’s area of competence, as set out within the Clinical
Directorate Service Plan and in line
with policies as specified by the Employer.
b) To
ensure that duties and functions are undertaken in a manner that minimises
delays for patients and possible disruption of services.
c) To
work within the framework of the hospital / agency’s service plan and/or levels
of service (volume, types etc.) as determined by the Employer. Service planning
for individual clinical services will be progressed through the Clinical
Directorate structure or other arrangements as apply.
d) To
co-operate with the expeditious implementation of the Disciplinary
Procedure
e) To
formally review the execution of the Clinical Directorate Service Plan with the
Clinical Director / Employer periodically. The Clinical Directorate Service
Plan shall be reviewed periodically at the request of the Consultant or
Clinical Director / Employer. The Consultant may initially seek internal review
of the determinations of the Clinical Director regarding the Service Plan.
f) To
participate in the development and operation of the Clinical Directorate
structure and in such management or representative structures as are in place
or being developed. The Consultant shall receive training and support to enable
him/her to participate fully in such structures.
g) To
provide, as appropriate, consultation in the Consultant’s area of designated
expertise in respect of patients of other Consultants at their request.
h) To
ensure in consultation with the Clinical Director that appropriate medical
cover is available at all times having due regard to the implementation of the
European Working Time Directive as it relates to doctors in training.
i) To
supervise and be responsible for diagnosis, treatment and care provided by
non-Consultant Hospital Doctors (NCHDs) treating patients under the
Consultant’s care.
j) To
participate as a right and obligation in selection processes for non-Consultant
Hospital Doctors and other staff as appropriate. The Employer will provide
training as required. The Employer shall ensure that a Consultant
representative of the relevant specialty / sub-specialty is involved in the
selection process.
k) To
participate in clinical audit and proactive risk management and facilitate
production of all data/information required for same in accordance with
regulatory, statutory and corporate policies and procedures.
l) To participate in and facilitate production of all data/information
required to validate delivery of duties and functions and inform planning and
management of service delivery
Service
Provision and Cover during Absences
The consultant has a continuing responsibility for the
care of patients in his/her charge and for the proper functioning of the
service allowing for appropriate delegation for the training of his/her staff.
He/she must be available by telephone and able to
attend the hospital in a timely fashion when on-call (40 minutes journey time).
Consultant staff must ensure that services are covered
during his/her planned absences. Where fixed commitments need to be cancelled
during planned absences appropriate notice (8 weeks) must be given. Absences
must be co-ordinated with other consultant staff in the specialty to ensure
senior cover is always available.
Essential Requirements
This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of
Paediatric – Haematology
Office Accommodation
The post holder will undertake administrative work
associated with their clinical and other professional duties. Adequate time and
facilities for clinical administration, including appropriate office space,
secretarial support and access to a personal computer, software & internet
access, will be available.
Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
