Overview
We are looking to appoint a
Consultant Geriatrician to undertake a fixed term position pending permanent
appoint for our University Hospital client in Connacht – Republic of Ireland.
You will work as part of a team of 8 Consultants providing excellent inpatient
care and Elderly Front Door Frailty.
Services
General in-patient services
The current consultants provide
care for the four elderly care wards a total of 48 beds. Orthogeriatric care is
provided with the assistance of a speciality doctor for the 2 acute orthopaedic
wards. There are also beds available on a dedicated Planned Investigation Unit
for elective admission to investigate general medical and related conditions. A
daily MDT white board meeting is held on each ward. The wards and ward rounds
are based on the ECIST SHOP/SAFER models.
There is regular input from the
Psychiatric Liaison nurses. The Day Therapy Unit, now housed in the Local Care
Centre, provides community support.
Out-patient services
Out-patients are seen in clinics
based in a secondary facility. Speciality clinics include a multidisciplinary
Falls clinic with associated Tilt Table Clinic. There are two Parkinson’s
Disease Specialist Nurses supporting the Parkinson’s service in the Community
as well as 2 hospital based PD Nurses who support inpatients across the
hospital. There are two Falls Specialist nurses with a dedicated Balance and
Stability group led by physiotherapists. Parkinson’s disease is currently
managed by Consultant Neurologists and Geriatricians. There are plans to create
a single referral pathway in the near future.
Rehabilitation
Community Healthcare includes the
specialist rehabilitation of the elderly currently centred at the Local Care
Centre co-located at secondary facility with the Stroke Rehabilitation Unit and
associated services, the Mental Health Services and a number of other community
oriented services.
Clinical work
The Elderly Care department at
the hospital wishes to greatly expand the scope of its current activity. There
are plans to develop an Elderly Frailty Unit located in the Ambulatory
Assessment Unit to rapidly assess and discharge older patients who do not
require admission to the acute hospital.
The department has other
priorities including improving cross-cover for consultant absence and seven day
consultant presence. All of these priorities require a significant increase
consultant resource.
Longer term, there are plans to
develop a specialised HCE ward for the care of patients with non-operative
trauma. This has come about due to increasing recognition that since the
hospital was awarded Major Trauma Centre Designation that the care of this
group of patients is complex and scattered across multiple wards in the
hospital. The department believes by cohorting them together we can achieve
best practice, better care and improved length of stay.
The postholder will need to
demonstrate excellent communication and team-working skills, being able to
liaise with nursing staff, therapists and discharge team as well as
communicating effectively with patients and their families. The postholder will
have excellent skills in acute medicine. The postholder will be expected to
demonstrate and evidence that their training and experience covers the entirety
of Geriatric Medicine to the advanced level expected of a Consultant
Geriatrician.
The postholder will be expected
to contribute to the acute medical take (currently 1:9 on-call) and elderly
care weekend ward rounds. The postholder will also be expected to contribute to
the provision of the Elderly Frailty Unit as well as a provide Rapid Access
Elderly Care Clinics and a daily Specialty Enhanced Take service.
There is flexibility within the
posts for individuals with departmental agreement to focus on specific parts of
departmental development for example non-operative trauma, surgical liaison
services, the Elderly Frailty Unit or developing community links and
out-patient services.

Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Medicine: Geriatric / Elderly Care Consultant, on behalf of
the Trust’s Medical Director, based on the provisional timetable. This will be
signed by the Chief Executive and will be effective from the commencement date
of the appointment.
It will be reviewed at 3 months & thereafter
annually or at any time, but no less than 3 months after a previous review, as
requested by the appointee, Operational Medical Director or Clinical Lead &
adjusted accordingly to the agreement of both parties.
An illustrative 10 PA job plan is shown below (this may vary depending on the needs of the unit
and
the experience of the post holder). The actual
timetable will be agreed with the Service Line
Director and where appropriate non generic SPA
time may be included for such things as formal
teaching, clinical supervision etc.

a) To participate in development
of and undertake all duties and functions pertinent to the Consultant’s area of
competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the
Employer.
b) To ensure that duties and
functions are undertaken in a manner that minimises delays for patients and
possible disruption of services.
c) To work within the framework
of the hospital / agency’s service plan and/or levels of service (volume, types
etc.) as determined by the Employer. Service planning for individual clinical
services will be progressed through the Clinical Directorate structure or other
arrangements as apply.
d) To co-operate with the
expeditious implementation of the Disciplinary Procedure
e) To formally review the
execution of the Clinical Directorate Service Plan with the Clinical Director /
Employer periodically. The Clinical Directorate Service Plan shall be reviewed
periodically at the request of the Consultant or Clinical Director / Employer.
The Consultant may initially seek internal review of the determinations of the
Clinical Director regarding the Service Plan.
f) To participate in the
development and operation of the Clinical Directorate structure and in such
management or representative structures as are in place or being developed. The
Consultant shall receive training and support to enable him/her to participate
fully in such structures.
g) To provide, as appropriate,
consultation in the Consultant’s area of designated expertise in respect of
patients of other Consultants at their request.
h) To ensure in consultation with
the Clinical Director that appropriate medical cover is available at all times
having due regard to the implementation of the European Working Time Directive
as it relates to doctors in training.
i) To supervise and be
responsible for diagnosis, treatment and care provided by non-Consultant
Hospital Doctors (NCHDs) treating patients under the Consultant’s care.
j) To participate as a right and
obligation in selection processes for non-Consultant Hospital Doctors and other
staff as appropriate. The Employer will provide training as required. The
Employer shall ensure that a Consultant representative of the relevant
specialty / sub-specialty is involved in the selection process.
k) To participate in clinical
audit and proactive risk management and facilitate production of all
data/information required for same in accordance with regulatory, statutory and
corporate policies and procedures.
Service
Provision and Cover during Absences
The consultant has a continuing responsibility for the
care of patients in his/her charge and for the proper functioning of the
service allowing for appropriate delegation for the training of his/her staff.
He/she must be available by telephone and able to
attend the hospital in a timely fashion when on-call (40 minutes journey time).
Consultant staff must ensure that services are covered
during his/her planned absences. Where fixed commitments need to be cancelled
during planned absences appropriate notice (8 weeks) must be given. Absences
must be co-ordinated with other consultant staff in the specialty to ensure
senior cover is always available.
Essential Requirements
This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of
Medicine: Geriatric / Elderly Care
Office Accommodation
The
post holder will undertake administrative work associated with their clinical
and other professional duties. Adequate time and facilities for clinical
administration, including appropriate office space, secretarial support and
access to a personal computer, software & internet access, will be
available.
Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
