Overview
Our University hospital client is
looking for an Orthopaedic Surgeon to complement their current arthroplasty
service and to take part in the trauma rota (currently 1 in 9). This is a
substantive / fixed term Orthopaedic Consultant post attracting 12.5 Pa’s
.Minimum fixed term of 12 months

registration and license to practice is an essential for this role. If you do
not currently have IMC registration, but you are in the process of applying,

registration at the time of shortlisting will not be considered- as per
hospital requirements.

Information
You will be able to take
advantage of the excellent Consultant contract for the Republic of Ireland. The
basic salary range for this post is euros €217,235 to €261,051 Euro’s per annum
plus a further euro 26,672 per annum available in Commitment Awards.
The
Orthopaedic Department
The Orthopaedic Department is
part of the Surgical Services Directorate, which is itself part of the Surgical
Division, within the University Hospital. The hospital offer orthopaedic
services to people living in the counties of Clare, Cork, Kerry, Limerick,
Tipperary, Waterford. Whilst the service is based in one hospital site, most of
our surgeons have clinics +/- theatre in community hospitals in the region.
Most orthopaedic conditions are
treated in house with specialist spinal services referred to the regional
trauma centre. There are currently 10 orthopaedic surgeons, 3 upper limb, 2
foot and ankle, 4 hip and knee surgeons and 1 Spinal/Paediatrics. The hospital undertakes
most facets of orthopaedic surgery including revision surgery.
They have an 20 bed
“Infection-free” ward for elective Orthopaedics. Hospital-acquired MRSA rates
are at an all-time low. Almost all in-patients for elective surgery are
admitted on the day through the Surgical Admissions Unit,
adjacent to the theatre suite, and are transferred to the ward post
operatively. In addition, there is a 20 bed Day Case Unit as part of the
theatre complex.
There are three ultra-clean air
laminar flow theatres. Two are dedicated orthopaedic theatres and one is used
for Trauma, Orthopaedics and by other specialties. There are 6 further
theatres.
The hospital has been designated
a Trauma Unit with the Regional Trauma Centre. The Department cares for head
and spinal injuries, with CT/MR scans, where patients are transferred if
surgery is required. The Trauma ward has 24 beds
Paediatric patients are nursed on
a dedicated children’s ward and admitted under joint care with the
paediatricians. The paediatric consultants are resident on-call.
The Orthopaedic Out-Patient Suite
includes a plaster room and orthotic service. There is an anaesthetist-led,
nurse-run pre-operative assessment clinic.
We have dedicated
musculo-skeletal radiology support including Ultrasound, CT & MRI.
Trauma
The Department runs a rolling
“Trauma Week” timetable, with the trauma week commencing on a Friday evening.
The duty Consultant is responsible for all trauma admissions and is on call
over the weekend (with 1 trauma list but no scheduled clinics). Monday to
Friday the Consultant runs the morning Trauma Meeting, carries out a ward
round, followed by the Emergency Clinic which starts at 10.00 am, (although we
are currently trialling a differing model of fracture clinic delivery). The
Consultant is also responsible for the PM Trauma List.
During the “Trauma Week”, all the
duty Consultant’s elective clinics and theatre lists are cancelled. Monday to
Thursday evening on-call duties are covered by colleagues on a 1 in 9 rota.
The appointed Consultant will be
responsible for the medical care of their patients and will be allocated duties
and responsibilities depending on their experience.
The appointed Consultant activity
will be a ratio of outpatient and surgical operative responsibilities relevant
to the needs of the individual and organisation.
Medical audit, ownership of the
Trust’s quality targets and continuing postgraduate education is a mandatory
part of the post. Study leave (up to 30 days in three years) will be available
to enable the post holder to attend meetings and courses in keeping with CME
requirements and subject to the conditions of the Hospital’s Medical Study
Leave Policy.
The successful candidate will be
required to undertake managerial duties associated with the care of their
patients and the running of the Clinical Department. Adequate office space and
secretarial support will be made available.
For 8 weeks out of every 9 you
will run an elective practice. Initially, the hospital are looking for you to
have a general workload but developing your lower limb practice, focusing on
the hip. Ideally, you would provide a comprehensive arthroplasty practice
including complex primary. They would hope you would support the revision hip
service currently run by two Consultant Surgeons.
Every 9th week you will do a
“Trauma Week”, as outlined above. You will also take part in the Monday to
Thursday night on-call rota, which is currently 1 in 8. This is non-resident
and runs from 1800-0800 each night.
There is a further, flexible 0.5
SPA.
One of your SPA’s is designated
for education & training. You will be expected to teach and assess medical
students, foundation doctors and specialty trainees. The other 1.5 SPA’s are to
be used for some or all of the following:

Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Surgery Consultant, on behalf of the Trust’s Medical
Director, based on the provisional timetable. This will be signed by the Chief
Executive and will be effective from the commencement date of the appointment.
It will be reviewed at 3 months & thereafter
annually or at any time, but no less than 3 months after a previous review, as
requested by the appointee, Operational Medical Director or Clinical Lead &
adjusted accordingly to the agreement of both parties.
The job plan equates to 12.5 PA’s plus an
additional 5% on call supplement. The 12.5 PA’s break down to 9 Direct Clinical
Care and 2.5 Supporting Professional Activity. The extra 1.5 DCC PA’s are not
mandatory but 2 other consultants are on 11.5 PA contracts by mutual agreement

Supporting Professional Activity (SPA) Programmed
Activities
It is expected that most consultants within the
Trust will have 2.5 PA’s for Supporting Professional
Activities (SPA).
1.5 SPA’s are “core” in order to support the
requirements for revalidation including participating in
audit, CPD and mandatory training.
Up to a further 1 PA will be available for SPA by
mutual agreement as part of the job planning process for further appropriate
verifiable SPA. This is granted for the first 12 months. If a consultant decides not to
undertake enough additional SPA either suitable alternative DCC PA’s are
offered or the overall PA’s will be reduced.
Some of the SPA will be expected to be delivered
flexibly and you will not always be expected to be present on Trust Property.
However, there will be some dedicated SPA time within the agreed job plan. This
is time you are expected to be available to The Trust.
a) To participate in development
of and undertake all duties and functions pertinent to the Consultant’s area of
competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the
Employer.
b) To ensure that duties and
functions are undertaken in a manner that minimises delays for patients and
possible disruption of services.
c) To work within the framework
of the hospital / agency’s service plan and/or levels of service (volume, types
etc.) as determined by the Employer. Service planning for individual clinical
services will be progressed through the Clinical Directorate structure or other
arrangements as apply.
d) To co-operate with the
expeditious implementation of the Disciplinary Procedure
e) To formally review the
execution of the Clinical Directorate Service Plan with the Clinical Director /
Employer periodically. The Clinical Directorate Service Plan shall be reviewed
periodically at the request of the Consultant or Clinical Director / Employer.
The Consultant may initially seek internal review of the determinations of the
Clinical Director regarding the Service Plan.
f) To participate in the
development and operation of the Clinical Directorate structure and in such
management or representative structures as are in place or being developed. The
Consultant shall receive training and support to enable him/her to participate
fully in such structures.
g) To provide, as appropriate,
consultation in the Consultant’s area of designated expertise in respect of
patients of other Consultants at their request.
h) To ensure in consultation with
the Clinical Director that appropriate medical cover is available at all times
having due regard to the implementation of the European Working Time Directive
as it relates to doctors in training.
i) To supervise and be
responsible for diagnosis, treatment and care provided by non-Consultant
Hospital Doctors (NCHDs) treating patients under the Consultant’s care.
j) To participate as a right and
obligation in selection processes for non-Consultant Hospital Doctors and other
staff as appropriate. The Employer will provide training as required. The
Employer shall ensure that a Consultant representative of the relevant
specialty / sub-specialty is involved in the selection process.
k) To participate in clinical
audit and proactive risk management and facilitate production of all
data/information required for same in accordance with regulatory, statutory and
corporate policies and procedures.
Essential Requirements
This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of Surgery:

Office Accommodation
The post holder will undertake administrative work
associated with their clinical and other professional duties. Adequate time and
facilities for clinical administration, including appropriate office space,
secretarial support and access to a personal computer, software & internet
access, will be available.
Service Provision and Cover
during Absences
The
consultant has a continuing responsibility for the care of patients in his/her
charge and for the proper functioning of the service allowing for appropriate
delegation for the training of his/her staff.
He/she
must be available by telephone and able to attend the hospital in a timely
fashion when on-call (40 minutes journey time).
Consultant
staff must ensure that services are covered during his/her planned absences.
Where fixed commitments need to be cancelled during planned absences
appropriate notice (8 weeks) must be given. Absences must be co-ordinated with
other consultant staff in the specialty to ensure senior cover is always
available.
Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
