Overview

 

Do you want to make a real difference in rehabilitation in adult mental health services both in the community and inpatient? This post provides input to The Unit (a 12 bed specialist rehabilitation unit for men with complex needs); and to the Community Mental Health Assessment and Recovery Team. There is also the option of a session a week contributing to county wide leadership and development of rehabilitation in Munster. It is a full time post comprising of 10 PAs and will be split as follows: 4 PAs working in Rehabilitation; and 6 in the CMHART. There will be an expectation for the post holder to split their SPAs (2.5) evenly across these components.

In Munster there are additional consultant psychiatrists who work in roles split across CMHART and acute and urgent care, with work in the CMHART sectorised by GP practice. There is also significant specialty grade psychiatrist input to the Unit. There are regular trainee posts in Munster involving both psychiatric and general practice trainees.

Additional Information

You will be able to take advantage of the excellent Consultant contract for the Republic of Ireland. The basic salary range for this post is euros €217,235 to €261,051 Euro’s per annum plus a further euro 26,672 per annum available in Commitment Awards.

CMHARTs

 

These are multidisciplinary team serving local catchment GP surgeries. The staff have been organised by –

 

·       Psychosis, (including AOT and EIP work)

·       Non-Psychosis (including personality disorder)

 

The integrated Community Mental Health Teams consist of nurses, social workers, specialist psychological input and input from Occupational Therapy and support staff. Treatment is delivered according to cluster based treatment pathways: psychosis and non-psychosis. Consultants work across these pathways.

 

Work within the team includes assessment, short term intervention and recovery in close conjunction with care co-ordinators. Patients are referred into the service via single point of access.  The post-holders will be expected to provide direct care to patients and to play a major role in consultancy for team members. Where clinics are arranged the usual arrangement would be for new patients to be allocated 1 hour, and follow-up appointments 30 minutes, with allotted time for administration.

 

Senior medical staff working in CMHART are part of the multidisciplinary team working with a Recovery focused model for secondary mental health problems. They attend team meetings as appropriate, and are part of the leadership of the team. The model includes allocated time for consultation slots with care coordinators as well as attending reviews, joining complex case discussions and completing assessments. Consultants will not have a significant stand-alone outpatient follow-up role.

 

The hospital are transforming CMHART to ensure will deliver interventions to the right people and use recovery focused pathways and have a project plan to develop these modern services. This involves an initial assessment by a member of the assessment team – which includes some medical time. For patients within CMHART the model is for care co-ordinators to use the medical time for consultation (booked in 15 minute slots) and joint reviews (booked in 30 min slots).

 

Working in Munster the catchment area will be split using GP surgeries and referral rates to allow for equitable distribution based on allotted time. There will also need to be flexibility to arrange cross cover with Munster colleagues for annual leave etc.

 

The Unit

 

The Unit Centre is a 12-bed unit for men with complex needs. The unit values the uniqueness of each service user and is dedicated to promote individual goals and aspirations.  The Recovery model is the principal clinical model for the ward.   The service is a lockable, specialist rehabilitation unit which aims to deliver effective rehabilitation and recovery to men whose needs cannot be met by acute services.  It forms a comprehensive pathway from acute services through to eventual discharge from inpatient services into the community.  The service described aims to place people from Munster into a local service designed to meet individual needs. The length of stay in the unit can vary from 12 weeks to 18 months. The Unit provides the opportunity the opportunity to offer consistent care to individuals with highly complex and individualised needs: this can includes complex physical and mental health diagnostic re-assessments. It offers an opportunity to work in a highly collaborative way with service users as there is the opportunity to build a consistent and extended therapeutic relationship.  There is a major focus on quality of life and realistic preparations for discharge including the management of alcohol use and other complex challenges

 

The Unit facilitates rehabilitation, working alongside each person to develop their strengths. Length of stay is individual, and can be up to 2 years.

 

There is an associate specialist psychiatrist (0.8 FTE) on the Unit and a Multi-Disciplinary team including: nurses, a psychologist, and occupational therapist.


Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Psychiatry: Rehabilitation Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

In April 2015 all wards adopted a new structure of working based on lean principals called APP (Acute Admissions Pathway).   Across the inpatient system this increased through put by reducing inefficiencies. As a positive consequence the Trusts use of out of county acute beds reduced to almost zero and bed occupancy reduced to around 85 percent.  This means when people need to be admitted they can be quickly and nearer to home.  These improvements have been excellent for the quality of care that is provided to people in acute distress and were recognised in the 2016 Health Service Journal awards where the project reached the finals for the Acute Innovation award.

 

PICU

 

Psychiatric Intensive Care Unit (PICU) has 10 beds serving the whole of Munster region (it is on the same site as the Unit)

 

Liaison Psychiatry

 

The hospital have expanded our Liaison psychiatry services across the hospital this year.  The Liaison services provided in the East of the County and now have dedicated psychiatry time alongside an increased nursing workforce. In the South liaison is provided to General Hospital.

 

Crisis Team

 

The Crisis and Resolution Home Treatment Team is a 24 hour service providing home assessment and treatment as an alternative to hospital admission for people experience a severe mental health crisis. They work closely with inpatient services and CMHART to ensure that patients are treated in the most appropriate setting.

 

Primary Care Mental health Services – First Step (Adult IAPT and counselling services are available countywide, including self-referral. Discussions are underway with ICCs about fuller developing PCMH provision.

 

Early Intervention in psychosis

 

The Early intervention teams in Cumbria are integrated in to CMHART, but with county wide clinical leadership.  They are multidisciplinary, and work to HSE guidelines, including delivery of CBT-P, and BFT.

 

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery


Essential Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Psychiatry: Rehabilitation

Office Accommodation

The post holder will undertake administrative work associated with their clinical and other professional duties.  Adequate time and facilities for clinical administration, including appropriate office space, secretarial support and access to a personal computer, software & internet access, will be available.

 

Service Provision and Cover during Absences

 

The consultant has a continuing responsibility for the care of patients in his/her charge and for the proper functioning of the service allowing for appropriate delegation for the training of his/her staff.

 

He/she must be available by telephone and able to attend the hospital in a timely fashion when on-call (40 minutes journey time).

 

Consultant staff must ensure that services are covered during his/her planned absences. Where fixed commitments need to be cancelled during planned absences appropriate notice (8 weeks) must be given. Absences must be co-ordinated with other consultant staff in the specialty to ensure senior cover is always available.


Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
