Overview

 

This post will contribute to the provision of comprehensive and accessible cancer services across the Munster Region. Key elements of cancer care at the hospital include provision of excellent cancer services as near as possible to the patient’s home and to expand the treatment options available to patients locally across the Munster Region.

The post holder will be a key member of the relevant multidisciplinary teams. The post holder will play an active role in the management of cancer services on site.

Specific Duties

The successful candidate will be expected to participate fully into the consultant led integrated specialist oncology service across the Munster Region. This will mean that he/she will hold clinics in the region as well as once a week in the hospital. A flexible approach will be required as the service continues to evolve and further modernize, as additional new and replacement consultant appointments are made. The clinical duties are subject to review from time to time in the light of changing requirements.

Tumor sub-sites which will be included in a negotiated Job Plan include: urological cancer (prostate and bladder) and lung.

The successful candidate will develop his/her specialist interests and work with the appropriate multidisciplinary teams for both day to day management of patients, as well as in audit and in the development of patient pathways and protocols. It is expected that the successful candidate will be expected to attend their site specific MDT Meetings. As a senior employee of the HSE they will work in close co-operation with, and support other clinical, medical professional and managerial colleagues in providing high quality healthcare to the HSE’s patients. Integral to these responsibilities is the following: -

·       The provision of a first class clinical service;

·       Effective leadership to all staff engaged in the specialty;

·       Sustaining and developing teaching and research in conjunction with Organisational policies;

·       Undertaking all work in accordance with the HSE’s procedures and operating policies;

·       Conducting clinical practice in accordance with contractual requirements and within the parameters of the HSE’s service plans; and

·       Maintaining the confidence of business plans and development strategies formulated for the specialty, or the HSE.

Duties will include the planning, prescription and supervision of patients receiving outpatient and inpatient chemotherapy and radiotherapy for solid tumors.

The successful candidate will take part in the Consultant on call rota for the Munster Cancer Treatment Centre, which includes out of hours advice concerning review and treatment of patients as well as Oncology emergencies. This includes predictable ward rounds undertaken on Saturday and Sunday. This will also include provision of cover for consultant colleagues’ periods of leave.

As part of his/her supporting professional activity sessions, the successful candidate is expected to play a full part in Clinical Governance activities and postgraduate medical education within the service and this may involve taking a lead or acting as a deputy lead for a particular aspect of the Clinical Division’s governance and medical education work. The successful candidate is expected to participate in the development and regular review of policies, pathways and protocols and also to participate in multidisciplinary and multiagency audit. Consultants are expected to attend the HSE’s mandatory training programme. All consultant staff are appraised on an annual basis

Additional Information

You will be able to take advantage of the excellent Consultant contract for the Republic of Ireland. The basic salary range for this post is euros 120,000 per annum to euros 160,893 per annum plus a further euro 26,672 per annum available in Commitment Awards.

A full induction programme for the successful candidate will be provided before clinical duties commence.

Key tasks:

·       To provide specialist Oncological services to the population served with special reference to the common cancers and agreed appropriate oncology subspecialties and to be an active participant in each of these multidisciplinary teams.

·       To develop a site specialist and research interest on both hospital sites. This will include developing clinical research and entering patients into appropriate national and international studies.

·       To provide a weekly ward round for his/her oncology inpatients on hospital sites. In addition, ward round for any inpatient admission, when required.

·       To work closely with clinical consultant colleagues and to provide seamless cancer care for all patients.

·       To work closely with colleagues in other Divisions to assist in providing an efficient service for patients transferring between the different disciplines and during the patient journey.

·       To liaise with colleagues in Oncology, Haematology and Palliative Care to provide inpatient cover for oncology patients.

·       To develop and participate in collaborative initiatives, having a high regard for clinical effectiveness and to the practice of evidence based medicine.

·       To participate in audit and teaching.

·       To have responsibility for ensuring active participation in continuing medical education (CME).

Oncology services at the Hospital will be provided by a team of experienced clinical oncologists, medical oncologists and specialist palliative care teams working collaboratively, and all holding appointments at the relevant acute hospitals. Close collaboration will be essential in the management of chemotherapy patients and emergency cases.

The person appointed should expect from time to time to fit in with alterations in circumstances, such as changes in Consultant staffing, introduction of new services, expansion or reduction of existing services and implementation and review of Annual Programmes.

The successful candidate may be required to undertake occasional duties at other hospitals and health premises within the area. A detailed job plan will be agreed with the post holder on appointment and will be reviewed annually to take account of the changing needs of the service.

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Medicine Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

Direct clinical care sessions

 

1. Two outpatient clinic session at the Outpatients Department, Cancer Treatment Centre. These clinics will provide slots for new and review patients. Supported by a Speciality Doctor.

2. Two outpatient clinic sessions at the Outpatients Department. These clinics will provide slots for new and review patients.

3. Involvement in relevant MDTs linked to clinical specialties, some of which may, if necessary, be managed by videoconference.

4. Chemotherapy planning, prescription and supervision.

Final agreement on the timetable of commitments will be agreed on taking up the post and then be reviewed at least annually as part of the job planning process.

 

As per the HSE Consultant Contract, the full time job plan has 10 sessions, made up of 8 clinical sessions and 2 supporting professional activity sessions. In the HSE a standard consultant’s session is 3.75 hours i.e. 37.5 hours/week for 10 sessions. Newly qualified consultants are offered 7 clinical sessions and 3 supporting professional activity sessions for the first year in post.

 

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery


Essential Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of General Medicine: Oncology Radiation

Office Accommodation

The post holder will undertake administrative work associated with their clinical and other professional duties.  Adequate time and facilities for clinical administration, including appropriate office space, secretarial support and access to a personal computer, software & internet access, will be available.

 

Service Provision and Cover during Absences

 

The consultant has a continuing responsibility for the care of patients in his/her charge and for the proper functioning of the service allowing for appropriate delegation for the training of his/her staff.

 

He/she must be available by telephone and able to attend the hospital in a timely fashion when on-call (40 minutes journey time).

 

Consultant staff must ensure that services are covered during his/her planned absences. Where fixed commitments need to be cancelled during planned absences appropriate notice (8 weeks) must be given. Absences must be co-ordinated with other consultant staff in the specialty to ensure senior cover is always available.


Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
