Overview
Our client’s
hospital is a Major Trauma Centre for isolated head injuries and thus has close
links to the Helicopter Emergency Medical Service (HEMS). They are undergoing
significant further development as a tertiary referral centre for the region,
including additional critical care capacity.
Emergency
Care:
Emergency
Care at the hospital is provided in its Emergency Departments (EDs) across two
sites, By dint of their geographical locations, the departments see a broad
spectrum of pathologies across both urban and rural settings. Combined patient
numbers approach 250,000 and reach in excess of 170,000 type 1 patients per
annum. The larger of the two EDs at hospital is comprised of an 8 bayed
Resuscitation Room and a 25 bayed Majors area. The Resus Room has dedicated
trauma, stroke and paediatric bays due to persistent demand. In addition to
these areas are Majors LITE, Paediatric ED, Urgent Care Centre and a Rapid
Assessment and Treatment Area. The smaller ED at the secondary hospital retains
the more conventional structure of Triage, Minors, Paeds, Majors and Resus. The
present team of consultants have a keen interest in further developing the
following areas of the department: Trauma, Critical Care, Education, Research
and Paediatric Emergency Medicine. Our Paediatric unit underwent a
transformation in 2014 and our vision is to become one of regions leading
paediatric emergency units.
Ambulatory
Care:
Ambulatory
Emergency Care (AEC) started as a small advisory service to facilitate
discharges from the wards. This was initially a consultant advisory service,
whereby patients were discharged home from a ward and followed up as an
outpatient by our virtual ward team. Following from the success of this
service, the hospital have continued to
grow and now offer a seven days a week 08:00-20:00 consultant led service
taking direct referrals from the ED department and GP’s out in the community.
In developing our service even further, they have also recently joined the AEC
network. The AEC helps in managing patients that have been phoned through by
GP’s or direct referrals by the Emergency Department. The AEC is consultant led
and is supported by adequate medical, nursing and clerical staff. The unit has
the capacity to hold 20 patients and currently planning on expanding the
service to take 50 patients a day
Assessment
Units:
The hospital
has two assessment units. The Medical Receiving Unit (MRU) is a 30 bed unit,
situated at main site. The Clinical Assessment Unit (CAU) is a 15 bed unit
situated at the secondary site. Both have good sized seminar room where we have
programmed activities such as business meetings, junior doctor teaching with
daily case of the day and audit presentation. The consultant offices are
located within both assessment areas. The units are both within a minutes
walking distance from Emergency Radiology, ED Majors and Resuscitation and High
Dependency Unit (HDU), Intensive Therapy Unit (ITU) areas. All areas are fully
integrated to ensure a smooth and efficient experience for our patients.

Additional
Information
You will be able
to take advantage of the excellent Consultant contract for the Republic of
Ireland. The basic salary range for this post is euros 120,000 per annum to
euros 160,893 per annum plus a further euro 26,672 per annum available in
Commitment Awards.

vast improvements that we have achieved and will continue to achieve in our
emergency departments on our journey to outstanding.
During 2020/21
The emergency departments underwent a comprehensive program of re-structuring
that included the creation of the urgent care centre and establishing the
re-direction of services. Their improvements directive also resulted in a
restructured governance infrastructure, the creation of the academic unit and
the re-structuring of our education and training programs for both medical and
nurses workforce. Each consultant agrees a Job Plan annually as part of the
appraisal process with the Clinical Director of Emergency Care, on behalf of
the Chief Executive.
The
department supports individually designed job plans that fulfil the clinical
responsibilities while allowing for each consultant to develop and enhance
academic and other professional interests both within and external to the
trust. The Job Plan takes into account the requirement regarding the number of
fixed and flexible commitments.
Whilst it
also reflects as much as possible individual consultant’s special interests,
and on call commitment, priority is given to direct patient contact. There is
prospective cover for annual leave and study leave. This presently is
remunerated with payment for an extra PA. Sick leave cover will be covered by a
locum out of hours.

weekend, and 1 in 4 weekdays, weekend on call excludes Friday. At weekends the
consultant on call will be expected to provide 6 - hour shop floor presence of
direct clinical care on each of the 2 days.

Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Emergency Medicine Consultant, on behalf of the Trust’s
Medical Director, based on the provisional timetable. This will be signed by
the Chief Executive and will be effective from the commencement date of the
appointment.
It will be reviewed at 3 months & thereafter
annually or at any time, but no less than 3 months after a previous review, as
requested by the appointee, Operational Medical Director or Clinical Lead &
adjusted accordingly to the agreement of both parties.

On-Call Responsibilities:

a) To participate in development
of and undertake all duties and functions pertinent to the Consultant’s area of
competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the
Employer.
b) To ensure that duties and
functions are undertaken in a manner that minimises delays for patients and
possible disruption of services.
c) To work within the framework
of the hospital / agency’s service plan and/or levels of service (volume, types
etc.) as determined by the Employer. Service planning for individual clinical
services will be progressed through the Clinical Directorate structure or other
arrangements as apply.
d) To co-operate with the
expeditious implementation of the Disciplinary Procedure
e) To formally review the
execution of the Clinical Directorate Service Plan with the Clinical Director /
Employer periodically. The Clinical Directorate Service Plan shall be reviewed
periodically at the request of the Consultant or Clinical Director / Employer.
The Consultant may initially seek internal review of the determinations of the
Clinical Director regarding the Service Plan.
f) To participate in the
development and operation of the Clinical Directorate structure and in such
management or representative structures as are in place or being developed. The
Consultant shall receive training and support to enable him/her to participate
fully in such structures.
g) To provide, as appropriate,
consultation in the Consultant’s area of designated expertise in respect of
patients of other Consultants at their request.
h) To ensure in consultation with
the Clinical Director that appropriate medical cover is available at all times
having due regard to the implementation of the European Working Time Directive
as it relates to doctors in training.
i) To supervise and be
responsible for diagnosis, treatment and care provided by non-Consultant
Hospital Doctors (NCHDs) treating patients under the Consultant’s care.
j) To participate as a right and
obligation in selection processes for non-Consultant Hospital Doctors and other
staff as appropriate. The Employer will provide training as required. The
Employer shall ensure that a Consultant representative of the relevant
specialty / sub-specialty is involved in the selection process.
k) To participate in clinical
audit and proactive risk management and facilitate production of all
data/information required for same in accordance with regulatory, statutory and
corporate policies and procedures.
Essential Requirements
This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of Emergency
Medicine
Office Accommodation
The
post holder will undertake administrative work associated with their clinical
and other professional duties. Adequate time and facilities for clinical
administration, including appropriate office space, secretarial support and
access to a personal computer, software & internet access, will be
available.
Service Provision and Cover
during Absences
The
consultant has a continuing responsibility for the care of patients in his/her
charge and for the proper functioning of the service allowing for appropriate
delegation for the training of his/her staff.
He/she
must be available by telephone and able to attend the hospital in a timely
fashion when on-call (40 minutes journey time).
Consultant
staff must ensure that services are covered during his/her planned absences.
Where fixed commitments need to be cancelled during planned absences
appropriate notice (8 weeks) must be given. Absences must be co-ordinated with
other consultant staff in the specialty to ensure senior cover is always
available.
Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
