Overview

 

An exciting new opportunity has arisen for a Consultant Rheumatologist and General Physician to join the established Rheumatology and Acute Medical teams at a University Hospital, based in the Connaught Region- Republic of Ireland

 

You must be on the HSE Specialist Register (Rheumatology and General Medicine) with MRCP (UK) or equivalent at the time of interview. You will work within a well-established team providing outpatient, Day case and inpatient services to patients from hospital sites to patients in the region.  You will be responsible for assessing and managing patients with a range of rheumatological conditions, will contribute to the medical admissions rota, and share responsibility with 3 other GIM/Rheumatologists for the on-going management of base ward in-patients. The post will also include involvement in Undergraduate and Postgraduate Education, the opportunity to be involved with the Departments active research programme and the potential to develop leadership skills.

 

Clinical

 

A full range of outpatient and day case rheumatology activity is carried out in the Treatment Centre. The successful applicant will provide clinics for new and follow up patients. Clinics are booked to Royal College of Physicians guidelines.

 

The appointee will also be involved in working and supervising the activities of clinical nurse specialists. The expectation is for clinics to be delivered over 34 weeks per year to allow for leave and medical commitments. Typically for this role, this approximates to 350 new patients per year with a new to follow up ratio of approximately 1:4.

 

A full range of urgent and emergency medical patients are admitted to Acute Medicine on the hospital campus and the appointees will be responsible for the management of those patients admitted under their care. The consultant’s role is to continually assess and review patients as they are seen by the junior team thereby facilitating earlier ‘senior’ decision-making and high quality clinical care.

 

All duties within Acute Medicine are rostered but other clinical commitments must be timed so that they do not clash with these duties.

 

The consultants on C4/C6 provide a high level of input to a range of patients on the ward. In the service redesign, the majority of these patients will be speciality rheumatology. There will remain a small number of undifferentiated medical patients on base ward and surgical wards for which we provide outlier cover.

 

There is an expectation of daily senior input to facilitate management, discharge and supervision of the junior doctor team. Duties entail provision with Consultant colleagues of a comprehensive acute service; provision of reciprocal cover for periods of annual/study leave; and continuing responsibility for the care of patients in his/her charge and the proper functioning of the department.

 

These duties are subject to review from time to time in the light of the changing requirements of the service. If alterations to the described duties are required these will be mutually agreed between the appointee, their Consultant colleagues and the hospital.

 

The person appointed will be required to devote substantially the whole of his/her time to the duties of the post and give them priority at all times.

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.


Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Rheumatology Medicine Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

Any consultant who is unable, for personal reasons, to work full-time will be eligible to be considered for the post on a part-time basis; if such a person is appointed, modification of the job content will be discussed on a personal basis with the Trust in consultation with consultant colleagues.

The out of hours commitment currently includes1:10 weekends and 1:12 evening shifts- these are a mix of 1700-1900 with overnight on-call from home and 1900-2200 shifts. The remainder of the input to acute medicine is rota shifts Mon-Fri 8-5 doing a range of activity including post take ward rounds, acute GP admission area cover and telephone triage The overall PA input to acute medicine is 2.25 which is a reflection of the annualised input.

The post will initially contain 1.5 core SPA’s to support activities required for CPD needs. Additional SPA up to 2.5 total could be available subsequently for additional roles within the hospital subject to agreement, as part of the on-going job planning process. This would include educational supervision and additional teaching or management commitments.

Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and Rheumatology Medicine.

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
