Overview
Our clients University Hospital
is looking to appoint a full time (10.5 PA) Consultant Haematologist as part of
a team of seven consultants in the Leinster Region
The post is based in Leinster;
duties include a one in seven on-call attending rota, a weekly clinic, MDTs,
contribution to laboratory, MDT meetings, research and teaching; in addition
each member of the team contributes equally to the haematology service
(visiting 36 times averaged per year).
There is a large portfolio of
network and commercial studies open, with a team of research staff, and
opportunities to develop studies in conjunction with the Universities of
Leinster; –several such studies have been set up and recruited to by the existing
team. Extensive opportunities exist to develop clinical sub-specialisation at
local, regional, and national level where appropriate, as well as teaching at
under and post-graduate level.
The haematology unit is a level 3
facility, with level 1 services provided in a new purpose-built unit on a
regional site. There is a joint MDT between the 2 hospitals, serving a total
population in excess of 500,000..
The post holder will be expected
to work in general haematology and be able to manage malignant and
non-malignant haematology, coagulation, transfusion and haemoglobinopathy
patients. A background in research or
education would be desirable. Following appointment, the successful candidate
will be given the opportunity to take on additional management or teaching
roles and/or cultivate sub-specialist interests, to complement the existing
strengths of the department.

You will be able to take
advantage of the excellent Consultant contract for the Republic of Ireland. The
basic salary range for this post is euros €217,235 to €261,051 Euro’s per annum
plus a further euro 26,672 per annum available in Commitment Awards.
Haematology department at both hospital within the
Group
Clinical
Haematology
The Clinical Haematology service
at the hospital and secondary hospital is a joint service with a catchment
population of over 560,000 and provides a patient-focussed high-quality service
with excellent purpose built facilities supported by the regional Leukaemia
Fund charity. The primary site delivers Level 3 care including autologous stem
cell transplantation. Inpatients benefit from en-suite rooms in a 13-16 bedded
inpatient unit. The department diagnoses 350 new haematological malignancies
per annum and provides 450 inpatient admissions, 6300 day cases and outpatient
episodes on the primary site. Nine outpatient clinics per week are held at
primary site. Disease specific clinics are offered for myeloproliferative
disorders and allogeneic transplant follow-up. There are 7 WTE consultant
haematologists, 2 specialist registrars, 3 registrars, 2 advanced nurse
practitioners, 2 clinical nurse specialists and 1 CMT (Core Medical Trainee)
doctor, with secretarial support and a dedicated team of chemotherapy nurses.
On the secondary site there are 5
outpatient clinics per week, where visiting consultants attend on a fixed day
to provide patient continuity for outpatients or day cases. The haematology
service at secondary is level 1 and is supported by two clinical nurse
specialists, a specialty grade haematologist and an SHO doctor.
A weekly joint-multidisciplinary
team meeting is held across both sites over a videoconferencing link. Patients
requiring allografts are referred to other specialist centres and return for
their post-transplant care.
Laboratory facilities
The haematology laboratory at the
primary site delivers 900 blood counts per day; the laboratory is currently
being reconfigured. A full range of coagulation tests including factor assays,
and thrombophilia tests are available. Two flow cytometers are available for
research and clinical services. The molecular biology laboratory provides a
range of molecular tests including JAK-2, BCR-ABL quantitation, T cell receptor
studies, immunoglobulin gene rearrangements and offers DNA sequencing with
dedicated clinical scientist support. A blood transfusion laboratory provides
blood product support for the treatment of haemophilia and bleeding disorders
and is actively introducing full vein to vein electronic tracking of blood.
There is an active Patient Blood Management committee chaired by the lead
Haematologist for Transfusion and a hospital transfusion team comprising
consultant haematologist, specialty doctor in blood conservation, transfusion
nurse practitioner, blood transfusion manager, and haematology laboratory
manager.
Consultants dual report bone
marrow aspirates and trephines as part of a Specialist Integrated
Haematological Malignancy Diagnostic Service; expert flow cytometry and
genetics support is provided by the Haemato-Oncology Diagnostic
Service leading to a unified marrow report for all patients.
Specific Duties
Provision of a clinical and
laboratory haematology service at both hospital sites within the group. The
incumbent will provide clinical support at the primary site for 4 days per
week; all members of the consultant team provide one day per week covering the
secondary service when not on call (to a total of 36 days per year). On the
secondary site cover days there will also be a ward round and laboratory work.
There are three full ward rounds at primary site during each on-call week (1 in
7, including Sunday), laboratory support, rapid referral review and telephone
advice out of hours (for both sites). Other regular commitments include a
weekly morphology session, MDT meeting and time for clinic preparation /
administration, chemotherapy prescribing, planning and review of patients on
treatment.
During on call duties at both
sites workload will be cross-covered by consultant colleagues, so that a
Monday-Friday presence is maintained at secondary site. Emergencies admitted to
the secondary site out of hours are under the care of duty physicians with
telephone advice from RDE haematologists, as specified in the Service Level
Agreement (SLA).

Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Medicine - Haematology Consultant, on behalf of the Trust’s
Medical Director, based on the provisional timetable. This will be signed by
the Chief Executive and will be effective from the commencement date of the
appointment.
It will be reviewed at 3 months & thereafter
annually or at any time, but no less than 3 months after a previous review, as
requested by the appointee, Operational Medical Director or Clinical Lead &
adjusted accordingly to the agreement of both parties.

The hospitals are in a transitional phase of
workforce modernisation in part due to the implementation of the new consultant
contract, and aims to limit consultants’ job plans to a maximum of 10 PAs per
week. Consequently, this job plan is
offered on the basis of 10 substantive PAs, plus any additional temporary PA
allowance, which will cease on the addressment of workload intensity/service
demand by the giving of 3 months’ notice.
The supporting professional activities (SPAs)
allocation within the provisional job plan is an illustrative guide and will be
finalised following individual agreement with successful candidate. A
minimum of 1.5 SPAs will be provided within a full time contract, for a
consultant's personal development. Further discussion on supporting
professional activities will depend on the Trust's requirements and the
individual's particular expertise.
Standard Duties and responsibilities
a) To participate in development
of and undertake all duties and functions pertinent to the Consultant’s area of
competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the
Employer.
b) To ensure that duties and
functions are undertaken in a manner that minimises delays for patients and
possible disruption of services.
c) To work within the framework
of the hospital / agency’s service plan and/or levels of service (volume, types
etc.) as determined by the Employer. Service planning for individual clinical
services will be progressed through the Clinical Directorate structure or other
arrangements as apply.
d) To co-operate with the
expeditious implementation of the Disciplinary Procedure
e) To formally review the
execution of the Clinical Directorate Service Plan with the Clinical Director /
Employer periodically. The Clinical Directorate Service Plan shall be reviewed
periodically at the request of the Consultant or Clinical Director / Employer.
The Consultant may initially seek internal review of the determinations of the
Clinical Director regarding the Service Plan.
f) To participate in the
development and operation of the Clinical Directorate structure and in such
management or representative structures as are in place or being developed. The
Consultant shall receive training and support to enable him/her to participate
fully in such structures.
g) To provide, as appropriate,
consultation in the Consultant’s area of designated expertise in respect of
patients of other Consultants at their request.
h) To ensure in consultation with
the Clinical Director that appropriate medical cover is available at all times
having due regard to the implementation of the European Working Time Directive
as it relates to doctors in training.
i) To supervise and be
responsible for diagnosis, treatment and care provided by non-Consultant
Hospital Doctors (NCHDs) treating patients under the Consultant’s care.
j) To participate as a right and
obligation in selection processes for non-Consultant Hospital Doctors and other
staff as appropriate. The Employer will provide training as required. The
Employer shall ensure that a Consultant representative of the relevant
specialty / sub-specialty is involved in the selection process.
k) To participate in clinical
audit and proactive risk management and facilitate production of all
data/information required for same in accordance with regulatory, statutory and
corporate policies and procedures.
l) To participate in and facilitate production of all data/information
required to validate delivery of duties and functions and inform planning and
management of service delivery
Essential Requirements
This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of
Medicine - Haematology
Office Accommodation
The
post holder will undertake administrative work associated with their clinical
and other professional duties. Adequate time and facilities for clinical
administration, including appropriate office space, secretarial support and
access to a personal computer, software & internet access, will be
available.
Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
Service Provision and Cover
during Absences
The
consultant has a continuing responsibility for the care of patients in his/her
charge and for the proper functioning of the service allowing for appropriate
delegation for the training of his/her staff.
He/she
must be available by telephone and able to attend the hospital in a timely
fashion when on-call (40 minutes journey time).
Consultant
staff must ensure that services are covered during his/her planned absences.
Where fixed commitments need to be cancelled during planned absences
appropriate notice (8 weeks) must be given. Absences must be co-ordinated with
other consultant staff in the specialty to ensure senior cover is always
available.
