Overview
This post is an exciting
opportunity to work with both the Nephrology and medical team in our client HSE
Hospital. This advert is for 1.5 WTE post working a 40:60% split GIM to renal
medicine.
The Regional Centre is a
busy unit covering population of Connacht and covering all aspects of
nephrology, dialysis and transplantation. It has a large Home dialysis program
(both Peritoneal dialysis and Home HD), 700 patients on HD and carries out over
100 transplants per year. The Nephrology aspects of this job will cover out
patients clinics, nephrology procedures, education and teaching with MDT
involvement. The successful applicant with have the opportunity to develop
their area of interest within the region. The centre has many areas of
outstanding practice but we welcome the opportunity for further development and
will support the successful applicant in both service and clinical development.

The Medical aspect of
the Job will include participation in the general medical model working closely
with the acute medical unit to support the medical take. The Hospitals Acute
medical unit is amongst the busiest in the region and has a well-structured
medical model ensuring expert input into patient care early in the patients
pathway. The successful applicant will be able to help shape the future of the
combined specialist and general medical model within the hospital

General internal medicine:
The post holder
would also be part of the general medical team within the hospital providing
consultant level support for the Acute take. This would take the form of both
managing the acute medical take providing senior decision maker review early in
the admission pathway and post take ward rounds in the medical admission unit.
GIM shifts will be on a rotational basis and in the form of shifts. During
blocks of GIM cover the post holder will not have nephrology responsibilities
and their priority will be GIM. A GIM rota will be provided in advance and
nephrology commitment will be built around it. This is an exciting opportunity
to be part of a new GIM model in a busy acute site and to help shape the future
of the service.
Nephrology:
The post holder
will be responsible for nephrology, dialysis and acute/chronic transplant care
in out-patient environments as well as in the satellite facilities. It would be
an advantage for the post holder to be able to perform nephrological procedures
(line insertions, renal biopsies) and to teach and supervise the Unit’s junior
doctors in these procedures.
The service is continuously developing its subspecialties and a particular interest and expertise in any field would be desirable and could be supported.
The service, given its large geographical area, is currently divided by sub-region (based around each of the 4 satellite acute trusts) with an individual nephrologist(s) for each to maintain patient continuity as much as possible. On-going development and growth has increased the renal in-reach ward services in these sites and the post will facilitate further development.
Physicians from a nearby hospital run
nephrology and renal replacement (RRT) clinics at all of the satellite acute
trusts and RRT clinics at the satellite dialysis units. This post will take
joint responsibility for nephrology and renal replacement therapy, including
nephrology and AKI inpatients.
An indicative work load for this post-holder
would include shared responsibility for Outpatient clinics, Satellite dialysis,
transplant, chronic renal failure and nephrology patients. There is a
requirement for support for the renal admission unit, ambulatory clinics and in
reach services within the hospitals they support.
Clinics lists are
4 hours and will house 8 to 15 patients per list depending on new/FU and
neph/RRT breakdown. Clinic slots are generally 15-20 minutes per nephrology
follow-up, 20-30 minutes for a RRT follow-up and 30 mins per new patient.
The post is 10
PAs.
The formal job
plan would be 50% GIM and 50% nephrology. GIM shifts will be pre specified. It
is the intention that the nephrology aspect of the post will be discussed and
negotiated depending of the successful applicant but will involve outpatient
clinics.
Standard Duties and responsibilities
a)
To participate in development of and undertake all duties and functions
pertinent to the Consultant’s area of competence, as set out within the
Clinical Directorate Service Plan and in
line with policies as specified by the Employer.
b)
To ensure that duties and functions are undertaken in a manner that minimises
delays for patients and possible disruption of services.
c)
To work within the framework of the hospital / agency’s service plan and/or
levels of service (volume, types etc.) as determined by the Employer. Service
planning for individual clinical services will be progressed through the
Clinical Directorate structure or other arrangements as apply.
d)
To co-operate with the expeditious implementation of the Disciplinary
Procedure
e)
To formally review the execution of the Clinical Directorate Service Plan with
the Clinical Director / Employer periodically. The Clinical Directorate Service
Plan shall be reviewed periodically at the request of the Consultant or
Clinical Director / Employer. The Consultant may initially seek internal review
of the determinations of the Clinical Director regarding the Service Plan.
f)
To participate in the development and operation of the Clinical Directorate
structure and in such management or representative structures as are in place
or being developed. The Consultant shall receive training and support to enable
him/her to participate fully in such structures.
g)
To provide, as appropriate, consultation in the Consultant’s area of designated
expertise in respect of patients of other Consultants at their request.
h)
To ensure in consultation with the Clinical Director that appropriate medical
cover is available at all times having due regard to the implementation of the
European Working Time Directive as it relates to doctors in training.
i)
To supervise and be responsible for diagnosis, treatment and care provided by
non-Consultant Hospital Doctors (NCHDs) treating patients under the
Consultant’s care.
j)
To participate as a right and obligation in selection processes for
non-Consultant Hospital Doctors and other staff as appropriate. The Employer
will provide training as required. The Employer shall ensure that a Consultant
representative of the relevant specialty / sub-specialty is involved in the
selection process.
k)
To participate in clinical audit and proactive risk management and facilitate
production of all data/information required for same in accordance with
regulatory, statutory and corporate policies and procedures.

To participate in and facilitate production of all data/information required to
validate delivery of duties and functions and inform planning and management of
service delivery

Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Renal/
Nephrology Medicine Consultant, on behalf of the Trust’s Medical
Director, based on the provisional timetable. This will be signed by the Chief
Executive and will be effective from the commencement date of the appointment.
It will be reviewed at 3
months & thereafter annually or at any time, but no less than 3 months
after a previous review, as requested by the appointee, Operational Medical
Director or Clinical Lead & adjusted accordingly to the agreement of both
parties.
Requirements
This post is open to
doctors who hold Registration as a Specialist in the Specialist Division of the
Registrar of Medical Practitioners maintained by the Medical Council in Ireland
in the specialty of Clinical and Renal/ Nephrology Medicine.

Management Responsibility
All Consultants are
required to attend the monthly Directorate Meetings. Post holders will be
expected to share in administrative duties allocated by mutual agreement within
the Directorate. As part of the Consultant Team, the consultant will offer mentoring support and lead the
existing clinical team through; joint clinics, one-to-one support, individual
case management and complex cases, identifying development needs and suggesting
CPD (Continuous Professional Development) opportunities and responding to
clinical enquiries via e-mail and telephone.
