Overview

 

Our client is a HSE Trust - that is a large, integrated healthcare organisation.

With over 7800 staff and 970 in-patient beds, we are a large provider of Acute District General and Specialised Hospital services, as well as Community Services and Children’s Mental Health Services.

·       Women and Newborn Centre including 5 Operating Theatres, Consultant led maternity unit, midwife led birthing centre, Level 3 Neonatal Intensive Care Unit, general, emergency and specialist gynaecology services

·       Urgent Care Centre: A new purpose built building with a children’s assessment and observation area, and an on-site GP unit

·       Elective Orthopaedic Centre (28 beds and 5 operating theatres)

·       Endoscopy Unit

·       Paediatric Day Surgery Unit

·       Radiology services including CT

·       Ophthalmology Unit and dedicated Ophthalmology Theatres

·       Recently opened elective care centre incorporating facilities for day case surgery and short stay surgery, two theatres one of which is laparoscopic theatre and a procedure room. This facility will eventually include chemotherapy suite and enhanced endoscopy facilities.

 

The Post

 

This is an 11 programmed activity (PA) post, with 8.5 PA set aside for Direct Clinical Care (DCC); and 2.5 PA for Supporting Professional Activities (SPA). This post will contribute to the general medical on-call rota which is a 1:21 for overnight cover and 1:7 for weekend cover.

 

Responsibilities:

 

1. To take a shared role in the development of the geriatric service, in conjunction with the management team and consultant colleagues in Medicine for Older People.

2. To provide ongoing intermediate care of older patients on the wards at the Community Hospital and Acute Hospital in partnership with consultant colleagues.

3. To develop and support sub-acute rehabilitation for geriatric patients in community hospitals and community settings.

4. To provide an outpatient service in various locations

5. To provide continued clinical responsibility for the patients under his/her care in conjunction with colleagues in medicine for older people.

6. To take part in agreed provision of cover for consultant colleagues during leave.

7. To develop and promote services that minimise hospital stay and maximise opportunities for recovery and independent living.

8. To participate in medical audit and clinical governance.

9. To provide professional supervision, direction and training to junior medical staff.

10. To participate in postgraduate teaching

11. To attend meetings of the Directorate of Medicine for Older People, Rehabilitation and Stroke.


It is the responsibility of the appointee to ensure that they maintain their knowledge and skills to the appropriate level laid down for their specialities by the relevant professional bodies (e.g. Royal College, Speciality societies). The trust for its part supports CPD in principle and is committed to providing the necessary time and financial support for such activities, subject to agreed local policies. Discussions are on-going to further extend this service.


This post also carries a commitment to support the management of acute illness in older people with oncall commitment for acute/GIM, and care of a minimum of 20 acute older/general medical inpatients as per the attached indicative job plan.

 

The Department

 

Medicine for Older People services are throughout all 3 sites of the Trust with the acute stroke unit and two acute MfOP wards on the main site and step down and rehabilitation wards on all other  sites. The directorate is currently in the process of developing and strengthening its stroke pathway and frailty pathway, ensuring patients are provided with the correct standard of care in the right environment. The directorate also

provides ortho-geriatric support for the fractured neck of femur pathway.


The directorate provides a comprehensive service for older patients both in hospital and in the surrounding community and comprises of integrated acute services, with designated elderly out-patient and in-patient rehabilitation services.

 

Trainees are largely ward based and the new appointee will receive pro-rata support from the department’s medical trainees. There will also be support from specialist nurse in stroke and the two trainee advanced nurse practitioner posts .


The directorate provides care from the ED (frailty pathway and development of the use of CGA), through the 5 acute MfOP wards on the main hospital site (4 elderly medicine and 1 acute stroke unit), through to the 7 wards/ departments on the other 4 sites which specialise in rehabilitation and step-down care.

Outpatients clinics including, TIA and elderly medicine clinics are also provided.

 

Standard Duties and responsibilities

 

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses, MRCP candidates.

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead
Medicine- Elderly Care Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and Medicine- Elderly Care

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
