Overview

 

A full-time. 10 P.A, Haematology Consultant post is available at our client’s hospital based in the Munster Region.

 

This is a replacement post.  The successful applicant will join the five other consultants in this busy department covering out-patient work at the hospital and the combined in-patient ward at the secondary site. Of the six Consultants, three are based at the primary site and three at secondary site. There is cross cover for the two hospital sites on call and a rota for Consultant cover on the combined in patient ward (1 in 6).

 

If the successful candidate does not have a special interest in thrombosis then they will work one day per week at primary hospital site. There is a possibility that in future the main base for Clinical Haematology will be at primary site – if this happens then more of the clinical sessions for this post will move to the primary hospital from secondary hospital site..

 

Any applicant who is unable for personal reasons, to work full-time will be eligible to be considered for the post. If such a person  is appointed, modification of the job content will be discussed on a personal basis with the Trust in consultation with consultant colleagues.

 

The six Haematologists currently work in close collaboration over patient care and laboratory services at their base hospital.  Each has an area of special interest covering Haemostasis and Coagulation, Transfusion, Haemoglobinopathies and Paediatric Haematology and share general laboratory duties.   The appointee will be invited to negotiate an area of special interest within the team. This post will be primarily based at main site with additional commitments at secondary site. This currently consists of a rotation as Consultant for in-patient care at the main site (for one week in six) as well as a one in six on call rota.  Sub duties within the Laboratory will also be on a rotational basis.

 

 Department Profile

 Clinical

 

The Clinical Haematology service provides for the management of all Haematological disorders including the treatment of Haematological Malignancy to BSH level 3.  This includes the treatment of young patients with acute leukaemia, complex treatments for relapsed lymphoma and autologous stem cell transplantation for lymphoma and myeloma.  The transplantation programme is fully accredited by JACIE as a satellite centre . The current five Haematologists work in close collaboration over patient care, with each patient having a named Consultant.  Within the provision of services for malignant haematology, a consultant colleague has an interest in Paediatric Haematology (on a shared care basis with another region and in collaboration with the local Paediatricians). However, all the consultants see adult patients with any malignancy.  The department has its own haematology MDT but also participates fully by videoconferencing in the regional MDTs for lymphoma (weekly) and myeloma and leukaemia (fortnightly).

 

There is an Anticoagulant Service, for which the Lead Clinicians are the retiring post holder at the main site and another consultant at the secondary site. If the successful candidate does not have a special interest in thrombosis then a consultant colleague will work one day per week at main site to do clotting clinics/anticoagulation and the new appointee will work one day a week at secondary site doing a general haematology clinic. The anticoagulant clinics are primarily run by nurses and supported by a computerised system (RAID).  At main site the service provides anticoagulant control for approximately 1000 patients, but many cases are GP dosed and the Hospital service concentrates on difficult or unstable patients.  There are currently in excess of 3,000 patients registered on the system, where currently all patients are managed by the hospital service. There are expanding pharmacist led NOAC clinics at both sites.

 

The Hospital Transfusion Committee is video conferenced across the 2 sites and the hospital participates in SHOT and the Blood Stocks Management Scheme.  A Transfusion Nurse (four days) is in post at main hospital site and a second post, a BMS specialist, at the secondary hospital site.

 

Facilities for Clinical Haematology at Secondary Hospital Site

 

The Haematologists have a routine outpatient clinic (mainly for follow up and routine general Haematology referrals) on a Tuesday morning and a smaller general clinic on a Wednesday afternoon. There is also a myelo-proliferative clinics and a ‘thrombosis/bleeding’ clinic weekly on Wednesday afternoons at secondary hospital site. New patients with suspected malignancies or with other urgent problems are seen in the Day Unit.

 

The Haematology/Oncology Chemotherapy area, has facilities for the Consultants to review patients prior to the administration of their Chemotherapy. New patients can also be seen for assessment and investigation such as bone marrow tests. Patients requiring admission have to be admitted to Main hospital site and there is provision for emergency ambulance transfer. The Chemo Day Unit also provides a base for the other Clinical Nurse Specialists / Nurses/ Research Nurses and for complementary therapies.  The appointee will have an office in the Haematology lab where the two Haematology secretaries are also housed.  The Pharmacy has an up-to-date sterile supply suite to provide chemotherapy on site. CT, MRI and ITU are all on site.

 

 Facilities for Clinical Haematology at Main Hospital.

 

Full outpatient services are provided through the Cancer Care and Haematology Unit (CCHU). This is a purpose built and well established unit, which contains offices for haematology and oncology Consultants, CNSs, secretaries, clinical rooms and a chemotherapy area, staffed by 8.6 WTE chemotherapy trained nurses. The recent in-patients can be seen for close follow-up on discharge from the ward and for initial assessment in the case of emergencies (such as neutropenic sepsis).  The successful candidate will have a shared office in this building.

 

All haematology and oncology patients undergoing outpatient chemotherapy are managed in this area and there is onsite pharmacy for chemotherapy preparation. The Radiology Department has CT scanning facilities and MRI facilities. The Haematologists hold clinics on Monday, Wednesday and Friday morning for all patients other than emergencies.  There is a separate anticoagulation clinic, mainly for difficult cases, on a Tuesday afternoon. Urgent patients are seen as needed in the same building

 

Patients who require admission to hospital are admitted to the Haematology inpatient unit. The average number of haematology inpatients is 8.  Inpatients Unit has a total of 12 (of a total of 18) beds designated for Haematology, these include 6 single rooms all with en suite facilities and 4 with air filtration for neutropenic patients. The staffing on the ward is sufficient to have Haematology trained nurses present at all times and to give adequate cover for the management of acutely ill patients and for administration of more complex chemotherapy regimens.

 

Patient Numbers

•            New malignancies requiring urgent therapy – Secondary hospital site, 3-4 and Main Hospital Site 3-4 per week.

•            Autografts- 12-14 annually 

•            Total outpatient activity 2013-4 including anticoagulation 3197 new patients and 20,001 follow ups.

 

Duties and responsibilities

 

1.       Responsibility, in conjunction with consultant colleagues, for the provision of a clinical and laboratory haematology and Blood Transfusion service for the population of the Munster Region. Clinical provision encompasses inpatient, day-case and outpatient services for both malignant and non malignant haematological disease.

 

2.       Shared managerial responsibility on an agreed basis with colleagues.

 

3.       Provision of an out of hours ‘on call’ service with consultant colleagues.

 

4.       Participation in undergraduate/postgraduate medical/nursing/laboratory staff teaching as appropriate.

 

5.       The post-holder will engage with the requirements of NCAG with regards to the local acute oncology service

 

6.       Provision of mutual cover with consultant colleagues for periods of annual leave, sickness and study leave on an equitable basis.

 

7.       Participation in Clinical Audit and Clinical Governance procedures. The successful applicant would become the Haematology clinical governance lead and conduct continuous monitoring of mortality figures on the departments inpatients in accordance with the Trust’s policy.  There is an active Clinical Audit Department who provide support to internal audit programmes. There are regular audits performed within the Clinical Haematology department with presentations at the monthly educational session.

 

8.       The HSE will provide prescribed doctors with support to complete a regular annual appraisal and with the revalidation process.           

 

9.       The current study leave budget in the hospital is 1000 euros  per annum and it is expected that the post holder will be registered for CPD with the relevant medical Royal College.

 

10.    The hospital operates an annual appraisal system in accordance with the Trust Medical Appraisal & Revalidation Policy and the IMC Good Medical Practice Framework. Appraisal identifies personal and professional development needs, while reviewing the doctor’s work and considering the doctor’s contribution to the quality and improvement of service delivery. The annual appraisal and documentation forms the evidence needed to meet the requirements for IMC CPD.


Any concerns about a medical practitioner’s fitness to practise raised by patients or staff of the designated body or arising from another source are dealt with under the Maintaining High Professional Standards Policy found on the Trust intranet.


Standard Duties and responsibilities

a)       To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b)       To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c)       To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d)       To co-operate with the expeditious implementation of the Disciplinary Procedure 

e)       To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f)        To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g)        To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h)       To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i)         To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j)         To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k)       To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l)         To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery


Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Haematology Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.


Currently there are 5 Consultants including this vacant post, all full time. There are also two full time Specialist Registrar Haematology posts rotating (one at main hospital and one at secondary hospital), a full time SHO General Medicine rotational post and a full time House Officer post for haematology inpatients. There is support from a SHO General Medical trainee in 2 main hospital haematology clinics per week. There is also a shared Haem/ Onc SHO post at secondary hospital (0.6 wte Haem)

The following is a suggested 10 PA timetable for the Consultant although details will take into consideration the appointee’s views. Potential applicants wishing to work fewer hours per week are encouraged to discuss the post with the Lead Consultant.

On call will be on a 1 in 6 rota with with the current Consultants (attracts payment of 1.0 PAs for on call, making a total of 10.0 PA s all together).  Similarly the appointee will be the ward consultant, covering the inpatients, 1 week in 6.

All job plans will be subject to review and possible readjustment on an annual basis as per the Trusts Job Planning Guidance Policy. Where it has not been possible to reach agreement between the Clinical Manager and the individual Consultant on the content of the job plan, or if a Consultant disputes the decision to stop their pay progression in a given year, the Mediation and Appeals procedure will apply.  This will comply with  Schedule of the National Consultant Terms & Conditions 2004.

Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and Haematology.

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
