Overview

 

We are looking for a dynamic and enthusiastic Consultant to join our clients hospital team of Consultant Physicians in the provision of acute medical and general medicine services.

 

This is a full-time position but candidates wishing to work part-time or job-share are encouraged to apply.  If a part-time or job-share person is appointed modification of the job plan will be discussed with the appointee on a personal basis.

 

Medical Assessment Unit:

 

The Medical Assessment Unit (MAU) occupies 24 beds and works together with the Emergency department to facilitate smooth, seamless care for patients. The department is well supported by a multi professional team, including doctors, nurses, a social worker and allied health professionals. Each shift has a medical registrar , a Foundation Year 2 or Core Trainee (CT) or GP Specialty Trainee (GPST) and a Foundation Year HO working as part of the medical team. In addition there is a SHO and House Officer  in the unit during the weekdays.   When working in the Medical Assessment Unit there is no continuing ward duty for the junior doctors, allowing the teams to concentrate on the patients in the Medical Assessment Unit.

 

The core duty of the post holder is to ensure senior review and implementation of a management and discharge plan for all patients admitted to the MAU. Medical Assessment Unit and the Ambulatory care area, ward rounds are undertaken twice daily by the MAU Consultants supported by the junior doctors working on the relevant day/night shift.  The post holder will work with the rest of the team to provide clinical leadership, consultant review of patients, clinical supervision and training of junior doctors, triage of patients to the most appropriate speciality ward.  In addition the post holder will provide leadership for development of protocols to allow outpatient management of some of the conditions currently managed as inpatients. 

 

The opportunities for training and teaching are immense and comprise a particularly valuable part of this post.  In addition to training and teaching doctors, the post holder will also have responsibility for the development of advanced nurse practitioners.

 

The post holder will be part of the Consultant 1 in 11 on-call rota for General Medicine and be involved in stroke thrombolysis.

 

MAU Activity:

 

•            No of admissions 2015/16= 4458 [4409 direct admissions and 49 internal transfers]

 

•            Average Length of Stay [LOS] during 2013/14 = 1.6 days

 

•            Discharges within 24 hours = 1924 [based on ward start and end times entered on PAS].

                   

The AMBULANCE, URGENT CARE AND COMMUNITY SERVICES CLINICAL BUSINESS UNIT (CBU)

 

The Trust has five Clinical Business Units:

 

•            Surgery, Women and Child Health

•            Mental Health and Learning Disabilities

•            Clinical Support, Diagnostics and Cancer Services

•            Medicine and

•            Ambulance, Urgent Care and Community Services Clinical Business Unit. 

 

There are currently 4 medical ward areas: -

 

•            (28 beds)– General Medical/Endocrinology & Gastroenterology

•            (27 beds, 18 Respiratory and 9 Surgical)

•            Stroke and Rehabilitation Unit (56 beds)

•            Coronary Care Unit (CCU) (6 beds) and Cardiac Step/Acute (12 beds).  The Coronary Care Unit includes a temporary pacing room.

 

Main Duties:

 

General:

 

The working week is well structured, starting at 08:00 with a post-take ward round with the night team followed by an MAU consultant led ward round with the day team. The MAU consultant then reviews the morning admissions; further admissions after this time are managed using a rolling ward round system reviewing patients throughout the shift. Two set ward rounds operate at weekends with the on call consultant physician.

 

General duties include:-

 

•            Providing rapid assessment, diagnosis and initial treatment of emergency medical patients

•            Enabling an informed decision as to whether the patient requires admission or can be discharged home or to residential care with a plan of treatment

•            Ensuring correct and timely diagnosis of new admissions

•            Reviewing investigations and other results

•            Assisting junior teams in constructing a management plan

•            Advising on appropriate further investigations

•            Ensuring appropriate treatment is prescribed

•            Ensuring that resuscitation issues are addressed

•            Discussing with patients/relatives the current issues

•            Ensuring facilitation of early discharge planning

•            Reviewing patients within the rapid assessment and other clinics

•            Establishing and running an emergency outpatient clinic at the MAU to offer timely advice to GPs on patients not needing admission and who cannot wait six weeks to attend outpatients

•            Overseeing the Ambulatory Care service when required

•            Participating in the Consultant general medical emergency on-call rota (1 in 11) into include prospective cover for colleagues’ annual leave and other authorised absences.

 

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses & MRCP candidates

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead General Internal Medicine- MAU Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

 

The job plan, for 10 PAs will commence with 2.5 Supporting Professional Activities [SPA].  After six months in post the SPA allocation will be reviewed with the post holder to confirm there are activities in place to justify the 2.5 SPAs; any necessary adjustments will be made with a minimum of 1.5 SPA. SPAs to be undertaken during the normal working week and we could be expecting this to be undertaken on-site but if SPA is time shifted to evenings or weekends this could be undertaken at home.

 

Programmed Activities in Job Plan:

 

Direct Clinical Care includes emergency duties (including emergency work carried out during or arising from on-call); operating sessions including pre-operative and post-operative care, ward rounds, multi-disciplinary meetings (MDT), outpatient activities, clinical diagnostic work, other patient treatment, travel between sites and administration - 7.5 PAs.

 

Supporting Professional Activities (continuing professional development CPD, medical education, formal teaching, audit, job planning, appraisal, research, clinical management and local clinical governance meetings) -2.5 SPAs.

 

 On Call Payment:

 

The on call commitment is 1 in 11 and the appropriate supplement with 3% payable.

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and General Internal Medicine / Acute Medicine.

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
