Overview


Our client’s hospital provides acute healthcare services to a core catchment population of approximately half a million people living in Connaught and the surrounding area. They offer a range of more specialist services to a wider population, serving residents of Galway, Leitrim, Mayo, Roscommon, Sligo & Donegal.

Obstetric Anaesthetic Service

All inpatient maternity services for the hospital have been sited at the University Hospital since May 2002. The number of deliveries is over 5,500 a year. They have a regional block rate of approaching 40% and a caesarean section rate of 23%. There are two obstetric theatres, a four bedded high dependency unit and in addition to the delivery rooms in the Consultant led obstetric unit, there are 8 delivery rooms in the midwifery birthing centre on the floor below. There is an elective caesarean section list each weekday morning and an anaesthetic clinic on Monday afternoon when all patients for the lists are seen as well as any patients who wish to speak to an anaesthetist antenatally. There are currently a number of consultant anaesthetist PAs. An ODP is available to help the anaesthetist with all the theatre cases. Midwives assist the anaesthetist to site regional blocks for pain relief in labour and we are able to provide minimum motor block epidurals.

 

The Anaesthetic Department

 

The Department of Anaesthesia is part of the Directorate of Surgery, Anaesthesia and Critical Care and provides services throughout the trust including to elective and emergency operating theatres, obstetrics, critical care, preoperative assessment, acute and chronic pain, interventional cardiology dental sedation and ECT. The centre for elective surgery is where there are 6 operating theatres catering for General Surgery, Breast, Orthopaedics, Ophthalmology, ENT, Gynaecology and a busy Day Surgery unit. The hospital has one of the longest established Preoperative Assessment Units anywhere and this is key to the efficient running of theatres. Cardiopulmonary exercise testing is undertaken in conjunction with the department of respiratory medicine. The hospital is the acute site with 6 operating theatres and an 18 bed combined Critical Care unit located adjacent to the anaesthetic department in the main building. Complex orthopaedics, cancer surgery and high risk patients including vascular and upper GI surgery is undertaken and there are dedicated trauma and emergency theatres in addition.

 

Chronic Pain Service

 

This service is provided across the hospital by Consultant Anaesthetists as part of a multidisciplinary team with psychology and physiotherapy. Out-patient clinics are held on all sites, while procedures are done mainly at main hospital as day patients. New developments include a multi-disciplinary one stop clinic at a nearby hospital site.

 

The Post

 

This post is for a full time Consultant Anaesthetist for a leading hospital in the Connacht Region with an interest in Obstetric Anaesthesia.

 

Opportunities for development in areas such as perioperative medicine, regional anaesthesia, and preoperative assessment exist and the successful candidate is encouraged to develop a special interest. The current Consultant timetable is shown below with the theatre duties subject to agreement with the successful candidate. This timetable may need to change in the future because of changes in the service and may need to be changed before the postholder commences employment.

 

The posts all contain Flexible Sessions where the post holders will be required to cover any sessions within their competency. These are useful for the department in introducing flexibility into Consultant working and are useful for the individual in that they allow skills to be retained in a wider range of sub-specialities than would be in a fixed job plan.

 

This will include paediatric anaesthetic sessions in order to allow participation in the general on call rota. Out of hours working: On call 1 in 18 at weekdays only for anaesthesia including general, paediatric and obstetrics. (There is a separate ICU on call rota) There is a nominal cover for inpatients on the nearby site. There will be a 1 in 8 weekend trauma or CEPOD list. This list must be swapped if it coincides with a leave period but the rota will be flexibly worked. SPA’s above 1.5 are subject to taking on additional departmental and trust roles e.g. educational supervision etc.

 

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses & FRCA candidates.

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead
Anaesthetics- Obstetrics Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and Anaesthetics- Obstetrics

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
