Overview


This is a substantive post. Our client is seeking a highly trained individual committed to providing high quality anaesthetic care for undergoing elective, emergency and trauma surgery.  The appointment will be whole time.  Any consultant who is unable for personal reasons, to work whole time, will be eligible to be considered for the post.  If such a person is appointed, modification of the job content will be discussed on a personal basis in consultation with consultant colleagues.

This is a substantive post in the Clinical Management Group of Intensive care, Theatres, Anaesthesia, Pain Management and Sleep (ITAPS).

The Department of Anaesthesia provides General Anaesthetic support to the following surgical services: General Surgery, Vascular, Plastics, Maxillofacial, ENT, Paediatric Surgery, Ophthalmology, Orthopaedic trauma, Gynaecology and Obstetrics Anaesthesia services. Inpatient and Outpatient pain services.

Obstetric Anaesthesia Services. The maternity unit currently has in excess of 4000 deliveries per year and on call is shared between dedicated Obstetric Anaesthetists on a shared on call rota with other Obstetric Anaesthetists based at the hospital site.

Within the department there is office space, managerial and secretarial support and a number of Seminar rooms. Anaesthetists have a strong input into undergraduate and postgraduate training.

PRINCIPAL ELEMENTS

·     To provide Consultant Anaesthetic cover for elective and emergency surgery including preoperative assessment and postoperative care.

·     To support the provision of consultant delivered perioperative care.

·     Supervise and train junior medical staff.

·     Work with the inpatient pain team to provide safe and effective postoperative analgesia to patients.

·     Teaching, research and administration. It is expected that the successful candidate will be offered an honorary teaching role with the University Medical School. It is expected that the post holder will take an active role in the teaching of medical students and foundation doctors as well as anaesthetic trainees Any formal teaching/research duties taken up after commencement in post such as taking on the role of Educational Supervisor will be assessed for additional SPA time.

·     Develop the Anaesthetic Service along with the other Consultants at the hospital during monthly Departmental meetings. If the candidate takes on a formal management responsibility this will be assessed for either additional SPA time or additional dedicated management time.

·     Work with the Quality and Safety team to enhance patient safety.

·     Work flexibly to help cover the workload of the CMG.

 

Standard Duties and responsibilities


a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.


b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.


c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.


d) To co-operate with the expeditious implementation of the Disciplinary Procedure 


e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.


f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.


g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.


h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.


i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.


j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 


k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.


l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses & FRCA, MRCA candidates.

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead
Anaesthetics Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.


It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & ad~justed accordingly to the agreement of both parties.

8.5 PA’s are allocated to direct clinical care. 1.5 PA’s are allocated as a minimum to all consultants for SPA in order to support the requirements of revalidation, which include activities such as participating in audit, CPD and mandatory training and to support clinical supervision of undergraduates and postgraduate speciality trainees.

Continuous Medical Education is timetabled to ensure that the skills to cover several sub-specialties at night are maintained.

Given our current establishment level, it is anticipated that there will be opportunity for the successful candidate to contract for additional programmed activities on a temporary basis, up to the maximum level consistent with the Working Time Regulations (i.e. 12 PA’s maximum), through discussion and mutual agreement.

PROGRAMMED ACTIVITIES

The hospital is undergoing a significant period of service reconfiguration which will require significant flexibility in terms of base site for day time and out of hours work.

The successful applicant would be expected to work flexibly for the benefit of the department to enable it to meet its service delivery requirements.

From time to time there will be the need to make changes to the job plan.  As a minimum it will be examined yearly during appraisal.  It is expected that the successful candidate will be prepared to negotiate such changes for the benefit of clinical service delivery and the department as a whole.

 

Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and Anaesthesia.

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
