Overview


Applications are invited for the post of Consultant Paediatrician with a interest in Emergency Medicine at our clients HSE University Hospital.  This is to fill a recently vacated post.

 

The post is for 10 PA’s with a 1:11 on call rota in general paediatrics.

 

There are close working relationships between the Emergency Department (ED) and general paediatric department. This post is joint funded by ED and Acute Paediatrics and is an important link role.

 

The department has a strong commitment to both undergraduate and postgraduate teaching and the appointee will be expected to take part in this. The appointee will undertake child protection casework when it arises in ED.

 

The University hospital has a thriving and innovative Emergency Department with a focus on clinical excellence.  Demand for their service is steadily growing and they now see over 96,000 new attendances per year. The hospital are expanding their consultant workforce to 12 whole time equivalents (WT) Consultants in Emergency Medicine in order to meet this demand and to provide shop-floor presence 16 hours a day, 7 days a week.

In 2011 the Emergency Department opened its co-located Urgent Care Centre, and more recently the Trust has led the way in Ambulatory Care, with the establishment of a new service that has received national acclaim. The Emergency Department has a Clinical Decision Unit with well-established pathways and a multidisciplinary team. There is a dedicated Children’s Emergency Department and Children’s Ambulatory Unit.  In 2015 they opened a new Rapid Assessment area at the front door of the Emergency Department in order to support new ways of working in the approach to initial assessment.

 

This is an exciting time for the Emergency Department. The post holder will have the opportunity to help shape the future direction of the department and take part in hospital wide initiatives to improve Emergency Care and the patient experience across the whole of the acute pathway.

 

The Departments of Paediatrics & Neonates

 

General Paediatric Inpatients

The department provides a 24-hour emergency service.

 

A separate 24-hour paediatric Emergency Department (ED) opened in 2009 and is led by a paediatric consultant dually accredited in paediatrics and emergency medicine. The inpatient ward has 23 beds including 10 cubicles and 2 HDU beds. The beds are used flexibly for medical and surgical cases, although the bulk of usage is for medical cases.  The 2 bedded paediatric high dependency unit is located within the general paediatric ward. There are, on average, 2000 admissions to the ward per annum, of which approximately 10% are high dependency admissions.

 

A paediatric day assessment unit has been running successfully for 20 years supporting developing paediatric ambulatory services. The day assessment unit treats approximately 4000 children a year.

 

Out Patients

 

There is a dedicated children’s outpatient department, providing facilities for the comprehensive management of children not requiring hospital admission

 

Daily emergency referral (“10 -12”) clinics are held for general practitioners to obtain a specialist opinion on children with acute illness.  Around 3000 emergency referrals are seen a year.

 

The number of out patient referrals has risen substantially over the past 5 years. Around 13,600 children are seen each year.  In addition to the general paediatric and neonatal clinics, regular specialist clinics are held and are summarised below. Many of the specialist clinics are run by a multi-discipliniary team including specialist nurses, dieticians and psychiatrists. Some of the chronic disease clinics e.g haemoglobinopathy, diabetes & rheumatology are also run jointly by paediatric and adult consultants to ensure a smooth transition from paediatric to adult care.

 

The Emergency Department & Urgent Care Centre

 

The hospital’s Emergency Department (ED) serves the culturally and socially diverse area of munster. They see over 96,000 new attendances per year across the full spectrum of presentations.  Around 20% of attendances are children.  The department is part of the Urgent and Emergency Care Network, and is a designated Trauma Unit, as part of the Munster Trauma Network.

 

The Emergency Department forms part of a “hot floor” encompassing the ED, Ambulatory Care, Acute Admissions Unit, ICU, and Theatres.  They have a four bay Resuscitation area with one bay equipped for paediatrics. The Majors area has 15 cubicles and there is a separate area, or Urgent Care Centre, for minor injuries and primary care.  The Children’s Emergency Department has dedicated cubicles, a high dependency area, a treatment room and a separate children’s waiting room. An eight bed area adjacent to Majors houses the Clinical Decision Unit.

 

The department has its own dedicated imaging suite with plain radiography, CT and ultrasound. There is dedicated “hot-seat” Consultant Radiologist presence during office hours, and have recently updated their bedside ultrasound machine with a state of the art device.  A PACS system and dedicated high resolution screens allow for digital radiology viewing.  There is an Electronic Patient Record system for patient tracking, electronic notes, ordering and data collection. The department is equipped with a blood gas analyser and pneumatic specimen transport system for priority turnaround on laboratory investigations.

 

The Emergency Medicine Consultants have office accommodation in the corridor adjacent to Majors, as do departmental management and senior nursing colleagues.  There is a seminar room with up to date audio-visual facilities, a staff rest room, shower, locker and changing facilities.  There is dedicated administrative support within the department from the senior admin team.


Clinical Duties (guideline only)

  • To provide the consultant role in paediatric A&E
  • To support its effective and efficient running
  • To provide regular paediatric resuscitation training for the paediatric staff in liaison with the resuscitation department
  • To support cross site development of paediatric protocols, appropriate for A&E
  • To at all times, comply with the IMC’s guidance on ‘Good Medical Practice’ as amended or substituted from time to time.
  • To ensure that patient confidentiality is maintained at all times.
  • To undertake duties as defined within the attached job plan in so far as practicable and in agreement with your Clinical Director.
  • To be responsible and managerially accountable for the reasonable and effective use of Trust resources that you use and influence, and professionally accountable to the Medical Director.
  • On commencement to ensure personal attendance at the Trust’s corporate Induction (or arranged Consultant Induction).
  • To participate in the Trust’s Statutory & Mandatory training programme.
  • To be responsible for the Clinical Management of in and out patients under your care.
  • participate as a member of a multi-disciplinary team in the provision of medical care to patients
  • co-operate with such arrangements as are put into place to verify the delivery of all contractual commitments
  • co-operate with such measures as are necessary to ensure compliance with the requirements of the European Working Time Directive and related Irish legislation (39 Hours per week)


Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.

 

Job Plan

 

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Emergency Medicine Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.


It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of Clinical and Paediatric Medicine MRCPCH or equivalent.

Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
