Overview

 

Our Client a University Hospital is seeking to appoint a permanent Consultant Paediatrician in the Connacht Region. The Consultant will have responsibility for providing an Acute Paediatric service in conjunction with other consultant colleagues. This job will be in the acute setting with a full complement of acute 1:8 on call and 1:18 hot weeks.

Department

General paediatrics and community child health is fully integrated in the Division of Surgery and Women’s & Children’s Services at the University Hospitals.

The Paediatrics And Community Child Health Department

The Paediatric and Community Child Health Department provides services for children in the area and the adjacent areas of Connacht. The University hospital has 9 Paediatric beds (including an Adolescent Unit) and a 6 cot Level 1 Neonatal Unit, where there are facilities for neonatal intensive care. Approximately 2,100 births occur annually and the over 5 population of 30,000 is the fastest rising in the region.

One bed on ITU is available for children staffed by RN-Children’s Nurses where possible. This bed is available for stabilising children prior to transfer out, or for short-term (less than 12 hours) intensive care for children with single system problems. The local paediatric intensive care unit is based at another hospital within the region.

The Paediatric Ward is staffed and equipped to manage the usual range of paediatric emergencies including specialty problems and exacerbations of chronic disease.

The Neonatal Unit has three trigger ventilators, two CPAP Drivers, Vapotherms, Cerebral Function Monitoring, invasive Blood Pressure monitoring, cerebral ultra-sound and blood gas monitoring. It is staffed to manage up to 9 infants. Our Level 1 status means that we can look after infants, of 30 weeks gestation and above requiring short term neonatal intensive care.

The Children’s Outpatient Department is a newly opened dedicated Paediatric clinic area; general clinics and speciality multi-disciplinary clinics for diabetes, asthma, rheumatology, respiratory, allergy, Obesity, cardiology, SCBU follow up and growth are led by the Consultants in the directorate. Visiting speciality clinics in gastroenterology, paediatric surgery, paediatric cardiology, cystic fibrosis, paediatric neurology, Paediatric nephrology and genetics are led by Consultants from supra regional centres.

A 4-bedded Paediatric Assessment Unit (PAU) operates all year round to ensure immediate evaluation of referrals by senior medical and nursing staff and to avoid unnecessary admissions.

Each Consultant is provided with office space and the support of a Secretary, and has access to a computer including Internet and pathology/X-ray results reporting.

Community Services

Children’s community services are delivered off site at the community centre (CC) local to the hospital Child and Adolescent Psychiatry and Psychology services, provided by the reginal Mental Health, are based at Child and Family Centres at other HSE hospital sites.

Community Child Health Services

Community Child Health Services are combined with Acute Paediatrics in an integrated team, providing benefits for patient care relating to better liaison and communication, particularly with child abuse, disability, special needs and newer sub-specialties.

Child Protection Services

The issue of Child Protection being “Everybody’s Business” is given a very high profile in the hospital. Awareness of, and services for, child protection have had an increased multidisciplinary profile throughout the Trust, and this initiative has been strongly supported by the Unit Management Team. All new clinical staff receive mandatory awareness training at induction.

Specific Duties

·       Share with consultant colleagues the Acute Paediatrician workload;

·       Provide with the other consultants a mutual out of hours cover for annual, study and professional leave.

·       Out of hours responsibilities are 1 in 8 weeknights on call from home

·       Hot week/Weekend responsibilities are 1 in 8

·       Provision with Consultant colleagues of a service to the University Hospital, with responsibility for the prevention, diagnosis and treatment of illness and the proper functioning of the department;

·       To conduct cardiology clinics

·       Conduct General Paediatric clinics

·       Professional supervision and management of junior medical staff;

·       Responsibilities for carrying out teaching, examination and accreditation duties as required and for contributing to undergraduate, postgraduate and continuing medical education activity, locally and nationally;

·       Participating in medical audit, the hospital’s Clinical Governance processes and in CPD;

·       Involvement in research

·       Managerial, including budgetary, responsibilities where appropriate;

·       Where it is agreed between the parties, work on behalf of University Hospital or services provided by the hospital for other agencies, e.g. the prison service.

Teaching Commitments

·       regular commitments to ward based teaching of undergraduates.

·       participation in the lecture programmes for 2nd and 4th & 5th year medical students.

·       teaching of other groups as required including other specialty juniors and specialist nurses.

 

Job Plan

A formal job plan will be agreed between the appointee, the Operational Medical Director and Lead Paediatric: Cardiology Consultant, on behalf of the Trust’s Medical Director, based on the provisional timetable. This will be signed by the Chief Executive and will be effective from the commencement date of the appointment.

It will be reviewed at 3 months & thereafter annually or at any time, but no less than 3 months after a previous review, as requested by the appointee, Operational Medical Director or Clinical Lead & adjusted accordingly to the agreement of both parties.

 

Clinics start at 0900 or 1400 and are templated to last 3 hours. Reception and nursing support is available in all clinics. Most clinics also have Play Specialist support.

Hot Week / Consultant of the Week

 

The hospital run a Hot week system for the Daytime ward and SCBU cover. The consultant receives handover on Friday morning prior to their weekend on call and then is the Hot Week consultant for the following week (0900-1700).

 

On-call responsibilities

 

Out of hours responsibilities are 1 in 8 week-nights (Non-resident/on call from home) Weekends: 1 in 8 Weekends (Non-resident/on call from home) Hot weeks: 1 in 8 weeks Middle grade duties: For discussion during Job Planning

 

Standard Duties and responsibilities

a) To participate in development of and undertake all duties and functions pertinent to the Consultant’s area of competence, as set out within the Clinical Directorate Service Plan  and in line with policies as specified by the Employer.

b) To ensure that duties and functions are undertaken in a manner that minimises delays for patients and possible disruption of services.

c) To work within the framework of the hospital / agency’s service plan and/or levels of service (volume, types etc.) as determined by the Employer. Service planning for individual clinical services will be progressed through the Clinical Directorate structure or other arrangements as apply.

d) To co-operate with the expeditious implementation of the Disciplinary Procedure 

e) To formally review the execution of the Clinical Directorate Service Plan with the Clinical Director / Employer periodically. The Clinical Directorate Service Plan shall be reviewed periodically at the request of the Consultant or Clinical Director / Employer. The Consultant may initially seek internal review of the determinations of the Clinical Director regarding the Service Plan.

f) To participate in the development and operation of the Clinical Directorate structure and in such management or representative structures as are in place or being developed. The Consultant shall receive training and support to enable him/her to participate fully in such structures.

g) To provide, as appropriate, consultation in the Consultant’s area of designated expertise in respect of patients of other Consultants at their request.

h) To ensure in consultation with the Clinical Director that appropriate medical cover is available at all times having due regard to the implementation of the European Working Time Directive as it relates to doctors in training.

i) To supervise and be responsible for diagnosis, treatment and care provided by non-Consultant Hospital Doctors (NCHDs) treating patients under the Consultant’s care.

j) To participate as a right and obligation in selection processes for non-Consultant Hospital Doctors and other staff as appropriate. The Employer will provide training as required. The Employer shall ensure that a Consultant representative of the relevant specialty / sub-specialty is involved in the selection process. 

k) To participate in clinical audit and proactive risk management and facilitate production of all data/information required for same in accordance with regulatory, statutory and corporate policies and procedures.

l) To participate in and facilitate production of all data/information required to validate delivery of duties and functions and inform planning and management of service delivery

 

Service Provision and Cover during Absences

 

The consultant has a continuing responsibility for the care of patients in his/her charge and for the proper functioning of the service allowing for appropriate delegation for the training of his/her staff.

 

He/she must be available by telephone and able to attend the hospital in a timely fashion when on-call (40 minutes journey time).

 

Consultant staff must ensure that services are covered during his/her planned absences. Where fixed commitments need to be cancelled during planned absences appropriate notice (8 weeks) must be given. Absences must be co-ordinated with other consultant staff in the specialty to ensure senior cover is always available.


Essential Requirements

This post is open to doctors who hold Registration as a Specialist in the Specialist Division of the Registrar of Medical Practitioners maintained by the Medical Council in Ireland in the specialty of General Paediatrics: Cardiology

Office Accommodation

The post holder will undertake administrative work associated with their clinical and other professional duties.  Adequate time and facilities for clinical administration, including appropriate office space, secretarial support and access to a personal computer, software & internet access, will be available.


Management Responsibility

All Consultants are required to attend the monthly Directorate Meetings. Post holders will be expected to share in administrative duties allocated by mutual agreement within the Directorate. As part of the Consultant Team, the consultant  will offer mentoring support and lead the existing clinical team through; joint clinics, one-to-one support, individual case management and complex cases, identifying development needs and suggesting CPD (Continuous Professional Development) opportunities and responding to clinical enquiries via e-mail and telephone.
