Overview
We welcome candidates to apply
for this opportunity to join the Upper GI/General Surgical Team at our clients
HSE University hospital. The hospital has an ambitious programme of service
modernisation and a reputation for providing high quality services and would
like you to become part of this.
They are currently reconfiguring
the GI service across the region to provide General and Specialist Surgical
services across their main sites and the posts will involve working at both our
main hospitals. The team will provide Upper GI services to a catchment
population of approximately 350,000. Major elective surgery will be
concentrated on the main site but elective surgery, diagnostic work and
outpatient activity will be delivered in the secondary site with duties shared
between the team. Emergency duties will be provided by the extended team of
Consultants across both sites. Appointment to these posts will aid in our
ambition to provide a comprehensive emergency biliary service and provide Upper
GI specialist support to the Colorectal team.
The department has close links
with the Medical School offering interested candidates opportunities for
undergraduate teaching or the development of management roles within the
medical school.
Candidates must have a higher
medical qualification (FRCS or equivalent) and be on the HSE Specialist
Register for General Surgery, or be within Six months of obtaining their CCT,
CESR, or European Community Rights at the time of interview.

Information
You will be able to take
advantage of the excellent Consultant contract for the Republic of Ireland. The
basic salary range for this post is euros €217,235 to €261,051 Euro’s per annum
plus a further euro 26,672 per annum available in Commitment Awards.
Specific Duties
The General Surgery and Urology
Directorate provides the management structure for the following
Breast
& Plastic Surgery Service
Based in the Leinster region with
5 Consultant Surgeons and 3 Consultant Plastic surgeons, providing services to
the region including diagnostic and surgical oncology service,
immediate/delayed reconstruction and lipomodelling. The plastic surgery team work
collaboratively within the directorate
(abdominal wall reconstruction/perineal reconstruction post oncological
resection) and with other directorates (soft tissue cover for limb trauma,
dermato-oncology, opinion and management of complex wounds)
Vascular
Service
Based in the Leinster region with
outreach services in Louth, Meath, Monaghan, Longford, Westmeath, Offaly,
Laois, Carlow, Kilkenny, Wexford, Wicklow, Kildare, Cavan. Seven Vascular
surgeons provide this regional service including management of Carotid disease,
Aortic reconstruction (open and EVAR), Infrainguinal reconstruction, vascular
access service to the renal population, PICC/midline service and a full range
of services for venous disease.
General
Surgery
Service redesign is underway to
provide an emergency and elective
General Surgery service across the hospitals based on 18 Consultant surgeons
(10 Colorectal and 8 Upper GI) working as a single unit based at the main site
but with a proportion of emergency duties in secondary site (resident on call
element). A 1 in 18 on call with 50% of
on call duties at each site has been agreed. During the week a daytime 2nd
on call surgeon of the opposite specialty will provide specialist support to
the oncall surgeon and at weekends this support will be provided between the on
call surgeons for both sites. Cross referral of subspecialty cases is supported
and facilitated. Shared clinical governance sessions rotating between sites
occur 4 times per year. Elective surgery, outpatients and endoscopy will be
provided by all Consultants and
Associate specialists on the secondary site but all major elective
surgery will be carried out in the main site. Of necessity and as dictated by
specialist expertise & clinical need, some emergency cases from secondary site will require transfer to
main site (interventional radiology, access to emergency endoscopy, high risk
laparotomy cases, severe acute pancreatitis requiring intervention). All 18 GI
surgeons will have equitable access to endoscopy lists, operating lists and
outpatients in both our major sites. Some peripheral clinics will also be
shared between the GI surgeons.
Urology
A comprehensive service with 4
surgeons based in York and 4 Surgeons based in secondary site whom collectively
support the Diagnostic Urology service in addition to providing elective and
emergency services to the 2 sites.
Each of the services is supported
by site specific cancer specialist nurses, surgical nurse practitioners; junior
doctors (speciality doctors, specialty registrars, core trainees and foundation
doctors).
Teaching
Commitments
·
regular commitments to ward based teaching of
undergraduates.

participation in the lecture programmes for 2nd
and 4th & 5th year medical students.
·
teaching of other groups as required including other specialty
juniors and specialist nurses.
Job Plan
A formal job plan will be agreed between the appointee, the Operational Medical
Director and Lead Surgery: Upper GI Surgery Gastrointestinal Consultant, on
behalf of the Trust’s Medical Director, based on the provisional timetable.
This will be signed by the Chief Executive and will be effective from the
commencement date of the appointment.
Job plans will
be reviewed at least annually, following an appraisal meeting. The job plan
will be a prospective agreement that sets out a Consultants typical working
pattern, duties and responsibilities and objectives for the coming year. It
will cover all aspects of a Consultant’s professional practice including
clinical work, teaching, research, education and managerial responsibilities
and external commitments.
Additional
programmed activities may be offered to the post holder, up to a maximum of 2
for full time posts, taking the maximum total to 12. Any offer of additional
programmed activities will be based on the needs of the service and in line
with the Trust objectives and will be reviewed on at least an annual basis.
A core 1.25 PAs
of SPA time is provided per week for Consultant and SAS grades.
Private Practice
You should
ensure that any private practice commitments are discussed with your Clinical
Director and entered into your prospective job plan. Any change to these will
need to be discussed between yourself and your Clinical Director and recorded
in an amended job plan. Consultants will be required to ensure that they meet
the code of conduct for private practice at all times.
On-call Arrangements
1 on 18 general
surgery on call (shared with 7 other Upper GI Surgeons and 10 colorectal
surgeons) based in the main site. This equates to 1 in 9 on call overall as
duties are shared between the 2 main sites. In main site the on call is currently 3 day on call (Monday 0800 to
thursday 0800) followed by 4 days on call (Thursday 0800 to Monday 0800) a week
later with appropriate access to dedicated daytime CEPOD lists and emergency
biliary lists in addition to a 24hour emergency theatre. Support will be provided
by the opposite GI specialty during the weekdays and at weekends. In the secondary site on call duties would
involve 48 hrs (Monday 0800-Wednesday 0800), 48 hrs (Wednesday 0800 – Friday
0800) and 72 hrs (Friday 0800 – Monday 0800) over 3 separate weeks with Consultant to Consultant handover of all
cases at changeover times with
appropriate access to dedicated daytime CEPOD lists and emergency
theatres.
Standard Duties and responsibilities
a) To participate in development
of and undertake all duties and functions pertinent to the Consultant’s area of
competence, as set out within the Clinical Directorate Service Plan and in line with policies as specified by the
Employer.
b) To ensure that duties and
functions are undertaken in a manner that minimises delays for patients and
possible disruption of services.
c) To work within the framework
of the hospital / agency’s service plan and/or levels of service (volume, types
etc.) as determined by the Employer. Service planning for individual clinical
services will be progressed through the Clinical Directorate structure or other
arrangements as apply.
d) To co-operate with the
expeditious implementation of the Disciplinary Procedure
e) To formally review the
execution of the Clinical Directorate Service Plan with the Clinical Director /
Employer periodically. The Clinical Directorate Service Plan shall be reviewed
periodically at the request of the Consultant or Clinical Director / Employer.
The Consultant may initially seek internal review of the determinations of the
Clinical Director regarding the Service Plan.
f) To participate in the
development and operation of the Clinical Directorate structure and in such
management or representative structures as are in place or being developed. The
Consultant shall receive training and support to enable him/her to participate
fully in such structures.
g) To provide, as appropriate,
consultation in the Consultant’s area of designated expertise in respect of
patients of other Consultants at their request.
h) To ensure in consultation with
the Clinical Director that appropriate medical cover is available at all times
having due regard to the implementation of the European Working Time Directive
as it relates to doctors in training.
i) To supervise and be
responsible for diagnosis, treatment and care provided by non-Consultant
Hospital Doctors (NCHDs) treating patients under the Consultant’s care.
j) To participate as a right and
obligation in selection processes for non-Consultant Hospital Doctors and other
staff as appropriate. The Employer will provide training as required. The
Employer shall ensure that a Consultant representative of the relevant
specialty / sub-specialty is involved in the selection process.
k) To participate in clinical
audit and proactive risk management and facilitate production of all
data/information required for same in accordance with regulatory, statutory and
corporate policies and procedures.
l) To participate in and facilitate production of all data/information
required to validate delivery of duties and functions and inform planning and
management of service delivery
Essential Requirements
This post is open to doctors who hold Registration
as a Specialist in the Specialist Division of the Registrar of Medical
Practitioners maintained by the Medical Council in Ireland in the specialty of
Surgery: Upper GI Surgery Gastrointestinal
Office Accommodation
The post holder will undertake administrative work
associated with their clinical and other professional duties. Adequate time and
facilities for clinical administration, including appropriate office space,
secretarial support and access to a personal computer, software & internet
access, will be available.
Management Responsibility
All Consultants are required to attend the monthly
Directorate Meetings. Post holders will be expected to share in administrative
duties allocated by mutual agreement within the Directorate. As part of the
Consultant Team, the consultant will
offer mentoring support and lead the existing clinical team through; joint
clinics, one-to-one support, individual case management and complex cases,
identifying development needs and suggesting CPD (Continuous Professional
Development) opportunities and responding to clinical enquiries via e-mail and
telephone.
Service Provision and Cover
during Absences
The
consultant has a continuing responsibility for the care of patients in his/her
charge and for the proper functioning of the service allowing for appropriate
delegation for the training of his/her staff.
He/she
must be available by telephone and able to attend the hospital in a timely
fashion when on-call (40 minutes journey time).
Consultant
staff must ensure that services are covered during his/her planned absences.
Where fixed commitments need to be cancelled during planned absences
appropriate notice (8 weeks) must be given. Absences must be co-ordinated with
other consultant staff in the specialty to ensure senior cover is always available.
